2015 Art Saves Animals ART AUCTION QUALIFICATIONS AND DONATION FORM (one form per piece, please) All art must be original, signed and of appropriate quality to mirror the high standards of MHHS. All art must be framed or able to be exhibited with ease and simplicity, with little potential for damage. This form must be signed and included, indicating the size of the piece, art medium and display requests. All art should be accompanied by an artist identification card or “visual bio” in a format decided by the artist. A 5x7 card including the following information is also acceptable: Title of work and date of construction Name of artist Medium/materials used All work must be delivered by Friday, October 2, 2015 to MHHS in Menands. No work is accepted on consignment – all art donations become the property of MHHS. ------I am donating this piece of art or handicraft to the Mohawk Hudson Humane Society for the 2015 Art Saves Animals event. I agree my donation becomes property of the Society and they may use it in any way to maximize profit to the organization. I further understand my art must be framed or packaged to allow its display without additional cost to the Society. I will arrange for my donation to be delivered to the Society at no cost to them.
