STATE OF WISCONSIN Agreement Amendment Summary EFFECTIVE: JANUARY 1, 2005

The following areas of Your group agreement are herein amended/added to reflect the changes listed below:

PROFESSIONAL AND ADMINISTRATIVE SERVICES AGREEMENT (PASA)

 Change all instances of Medicare Plus $100,000 to Medicare Plus $1,000,000 within the PASA

HEALTH BENEFIT PLAN

 Article I Definitions adds the following definition:

SELF-ADMINISTERED INJECTABLE: Means an injectable that can be safely self- administered by a layperson. This does not include those drugs delivered via IM (intramuscular), IV (intravenous) or IA (intraarterial) injections or any drug administered through infusion.

 Article VII is deleted in it’s entirety and replaced with Attachment A.

 Article VIII is renamed Standard Plan and SMP Benefits for Wisconsin Public Employers. Section D. and following is added with Attachment B.

 Change all instances of Medicare Plus $100,000 to Medicare Plus $1,000,000 within the Health Benefit Contract.

Article IX. MEDICARE PLUS $1,000,000 add the following: C. BENEFITS AVAILABLE 5. PROFESSIONAL SERVICES and OTHER SERVICES o. BENEFITS are payable for CHARGES for initial preventive physical examination for individuals whose coverage begins on or after January 1, 2005. This is defined as PHYSICIAN’S SERVICES consisting of a physical examination (including measurement of height, weight and blood pressure, and an electrocardiogram) with the goal of health promotion and disease detection; includes education, counseling, and referrals for specified screening SERVICES and other preventive SERVICES. It does not include clinical laboratory tests. Any payable preventative exam must be performed no later than 6 months after the individual’s initial coverage date in accordance with the requirements under Part B of MEDICARE.

Article IX., C., delete 7. (Aggregate Lifetime Maximum Benefit Limit) in its entirety and replace with the following: 7. Aggregate Lifetime Maximum Benefit Limit The aggregate lifetime maximum BENEFIT limit for BENEFITS paid for CHARGES for SERVICES and supplies covered under this Article IX is $1,000,000 per PARTICIPANT during the lifetime of the PARTICIPANT while that PARTICIPANT is covered under this CONTRACT.

Article IX., C., 8. Exclusions All exclusions set forth in Article XIV of the CONTRACT apply to this Article IX. In addition, the following SERVICES are excluded from BENEFITS, except as otherwise specifically provided.  Article XIV exlusion LL is changed to:

LL. Charges for injectable medications. Except for Self-Administered Injectable medications; and injectable and infusible medications administered during home care, office setting, confinement, emergency room visit or urgent care setting.
