This post is based in the CAMHS Looked After & Accommodated Team (CAYP CAMHS for Accommodated Young People). First level registered nurse, AHP, Social Worker or child trained Clinical Associate in Applied Psychology (CAAP), providing specialist psychological therapies service to children, young people, their families and carers with mental health problems referred to secondary or specialist mental health services. The post holder is responsible for, and has a high level of autonomy in the assessment, formulation, prescribing and delivery of psychological therapies treatment and care to children, young people, their families and carers on a defined case load, with decision making responsibility for evaluation and discharge from treatment or referral on to other agencies. You work as a member of the multidisciplinary team and have a responsibility to provide clinical supervision to other qualified practitioners and practitioners in training. You also have a responsibility for training and education of others and actively planning and participating in research within the clinical service. On occasion the post holder will be expected to carry out delegated tasks of the senior clinician providing local coordination in their absence.

Line Management: Profesional Leadership: 4. SCOPE AND RANGE

 Responsible for the autonomous assessment, formulation, prescribing and delivery of specialist psychological therapies to young people, their families and carers with mental health problems on a defined case load.

 Based within a multidisciplinary CAMHS team providing a psychological therapies service, post holder has responsibility for clinical decision making for patients on case load in relation to discharge of patients from treatment or referral on to other agencies

 Provision of professional leadership and supporting the locality coordinator in appraisal of other practitioners in the multidisciplinary team as appropriate.

 Provision of specialist expertise and advice to NHS Lanarkshire staff, carers and other external agencies on aspects of psychological therapies treatment for a range of clinical conditions

 Where required the post holder will undertake a full range of duties of the locality coordinator in their absence.

 Supports the locality coordinator or head of department in setting and monitoring standards of care, treatment and service provision, on an ongoing basis, through effective clinical effectiveness systems and processes.

5. MAIN DUTIES / RESPONSIBILITIES

Practising within the eKSF, scope, range and competencies of the post

 Responsible for the autonomous assessment, formulation, prescribing and delivery of specialist psychological therapies to young people, their families/carers with mental health problems on a one-to-one or group basis. Responsibility for clinical decision making for patients on case load in relation to discharge of patients from treatment or referral on to other agencies

 Deliver evidence based psychological therapies as identified through the National Psychological Therapies Matrix

 Implement Integrated Care Pathways for individual patients as appropriate

 Support and assist the Team Manager locality coordinator / head of department/senior

 Maintain records in accordance with professional body and organisational standards.

 Act autonomously to resolve inadequacies, which may be to the detriment of the client’s welfare and safety and report to the locality coordinator / head of department / line manager, where appropriate.

 Actively and meaningfully involve relevant people, and collaborate with the patient and carers in a shared responsibility for assessment, prevention and management of risk and positive risk taking, and individual vulnerabilities within the scope, range, skills and competencies of the post

 Work to maximise social inclusion by promoting awareness of, and coordination of social inclusion activities, supporting patients to access community organisations and supports according to their individual abilities and choices, ensuring they are incorporated into the patient care and treatment plan

 Empower young people, their families and carers to take responsibility for their health, well- being and future lifestyle as appropriate by practising in an open, transparent and inclusive manner, identifying their strengths and sources of personal social supports, promoting and respecting their rights, ensuring that patients have the relevant information to participate in the decisions about their care and recovery.

Team Working / Clinical Leadership  Deputise for the Team Manager, Locality Coordinator / Head of Department / Line manager as appropriate and act to ensure the effective running of the service in their absence as required.

 Ensure that changes in circumstances, which may affect delivery of care are managed and reported to the Locality Coordinator / Head of Department / Line Manager.

 Participate in the ongoing development of the service and the development of links with other areas of service delivery, voluntary and statutory services within Lanarkshire.

 Develop clinical/nursing practice in line with NHSL guidelines/ policies/ protocols/ procedures / best practice statements; Nursing and Midwifery Council requirements; National Quality Standards and Scottish Government Health Department policy and guidance.

 Respect at all times the confidential nature of your work and the rights and choices of patients and their carers in regard to confidentiality, ensuring patients are aware that certain information may have to be shared with other members of the clinical team or other authorities where there is concern for the health, safety or welfare of the patient or others, in accordance with guidance and protocols of the Lanarkshire Data Sharing Partnership.

 Ensure young people, their families and carers have sufficient information to consent to any aspect of care and treatment. If capacity to consent is impaired ensure additional assessment, input or legal safeguards are initiated.

 Manage the health and safety of staff, patients and carers within the service and report any health and safety deficiency as appropriate to the Locality Coordinator / Head of Department / line Manager. Ensure safe working environment and practices in accordance with Health & Safety legislation and compliance with locally agreed protocols.

 Apply the principles and safeguards of legislation, relevant to the clinical area and patient group, to support the care and treatment of patients, carers and dependents

 Ensure compliance with Lanarkshire Data Sharing Partnership policy and guidance, and information governance standards regarding accessing, sharing, recording, use and storage of information

 Implement all relevant NHS Lanarkshire clinical, HR and organisational policies

 Implement, maintain, monitor and audit evidence based clinical standards and standards of nursing practice to ensure that practice is based on research evidence available and in line with national quality standards.

 Participate in developing and implementing local policies which may impact on other members of the multidisciplinary team / other service areas by contributing to the work of local working/steering groups as agreed with the locality coordinator / head of department / line manager

 Implement and monitor clinical/nursing policies and procedures in line with NHSL guidelines, NMC requirements, other professional bodies (i.e.. BABCP), Scottish Government policy and guidance, legal requirements, emerging evidence and changing practice.

 Responsible for the delivery of a system for supervision and performance appraisal of junior staff within the team as appropriate.

 Involve patients and carers in monitoring and development of services.

 Attempt to resolve informal complaints as they arise and assist the locality coordinator / head of department / line manager in gathering data / resolving complaints which progress via the formal complaints procedure.

 Reflect on professional and clinical performance, including the emotional impact on self of working with people with moderate or severe mental health problems and engage in regular clinical supervision in accordance with NHS Lanarkshire clinical supervision guidance and policy and standards set down by professional psychological therapies bodies i.e. BABCP.

 Assist the organisation to achieve its performance objectives, including the delivery of HEAT Targets particularly in relation to access and waiting times targets for CAMHS and psychological therapies, Crisis Standards, Rights Relationships and Recovery Actions, National Quality Standards, National Inspectorate standards and MWC recommendations.

Training and Development  Actively contribute to the development of an atmosphere and environment, which promotes and facilitates learning, regularly liaising with Practice Education Facilitators, Clinical Director and Senior CAMHS Clinicians to support learning and practice development in the clinical area

 Actively participate in the development and delivery of specialist undergraduate and post graduate training programmes with colleagues in Higher Education Institutions.

 Act as a resource for other disciplines to ensure that contemporary information is available on clinical practice issues.

 Undertake appropriate continuing professional development as identified within the personal development planning process and eKSF

 Participate in all mandatory and in-service training/study days as required in response to patient and service need.

 Ensure contribution to the evidence base within the clinical field by actively designing and participating in research, and sharing good practice through networks, conferences and publications

Public Health/Health Promotion  Participate in the identification of health needs of the local population accessing the service.  Promote a healthy lifestyle and well-being providing advice to young people, their families / carers in line with corporate health promotion strategy and mental health and wellbeing strategy

 Demonstrate knowledge of the determinants of ill health and appropriate interventions. Be aware and give advice on issues relating to health education/promotion and health improvement.

7. SYSTEMS

 Use of Information technology to input and produce data and reports (Word, Excel, Email etc, Electronic Patient Information Systems), ensuring compliance with data protection policy, standards for information governance, and guidance on information sharing from the Lanarkshire Data Sharing Partnership  Systems for manual recording of client / patient relevant clinical / non clinical information  Systems for routine data collection and performance monitoring  Implement and monitor a range of systems including capacity planning tools, care planning systems, patient reporting systems, review notes, assessment tools, recording of treatments, orders, and stores orders etc.  Implement the agreed NHS Lanarkshire systems and processes for clinical supervision, mentoring, and eKSF / PDPs  Utilisation of Integrated Care Pathways  Ensure compliance with all systems related to regulatory codes of practice and legislation e.g. Mental Health Legislation, AWI, CPA, MAPPA, NMC registration; BABCP  Participate in ensuring robust systems are in place to monitor issues related to relevant legislation.

8. DECISIONS & JUDGEMENTS

 Exercise autonomous responsibility for clinical decision-making and judgement in response to assessed needs on very complex cases that require a great degree of analysis of to maximise the health, safety and welfare of the patient and others.

 Ensure effective liaison and consultation with the multidisciplinary care team as appropriate.

 Take delegated charge of the service in the absence of the locality coordinator / head of department, making judgements and decisions on the effective running of the service and delivery of care in their absence, seeking advice from line manager and other colleagues as required

 In the absence of the locality coordinator / head of department, delegated responsibility for changes to staff work plan in response to clinical need / risk or unexpected staff absence seeking advice from service manager as appropriate

 Participate in joint working, and organise / participate in joint working processes with multi- professional colleagues, making judgements and decisions from a professional perspective e.g. Child protection, Risk Assessment following DSH.

 Make judgements and decision based on highly complex information independently and in association with other members of the multidisciplinary team, seeking advice and consultation when necessary

 Responsibility for personal / professional development planning and appraisal of junior staff.

 Delegated responsibility for the ordering of stocks and supplies, ensuring adequate supplies for the requirements of the service as appropriate.

9. COMMUNICATIONS & RELATIONSHIPS

 Responsible for the development and maintenance of effective liaison, communication and close working relationships with colleagues in health services and other statutory and voluntary service providers.  Responsible for the development of therapeutic alliance / relationships with patients and carers  Communicate highly complex information regarding the management of a range of mental health problems to colleagues and other disciplines / professions, patient and carers in a manner that is clear and easily understood.  Communicate with patients and their relatives using a variety of communication skills, for example collaboration, active listening and communicating with patients with impaired communication skills due to their physical/mental health, ensuring access to interpreters or communication aids as required.  Develop and maintain effective communication systems and processes and working relationships with other professionals, carers, families and relevant others relevant to the patients health and welfare.  Produce written reports, papers and plans as required.  Provide verbal and written information within the clinical team and across teams and services to ensure effective communication between clinical staff, clinical teams and line managers  Work as a fully integrated member of a multi-professional team.

10. DEMANDS OF THE JOB (physical, mental, emotional)

Physical  Combination of frequent changes between walking, sitting and standing  Physical management of aggressive incidents including de-escalation, and breakaway of patients. This requires formal management of aggression training  Physical transportation of case files and documentation to community clinics  Driving as appropriate to community based venues, home visits, clinics etc.

Mental Demands  Majority of time is spent in face to face intensive interventions with children, young people and their families/carers who have a range of mental health problems which are complex in nature. This involves lengthy periods of sustained and intense concentration whilst delivering evidence based practice e.g. whilst carrying out appointment sessions and group interventions.  Routinely working with young people, families/carers who present with complex and challenging, disturbed or risky behaviours. The changing nature of patients mental and emotional state during individual sessions requires the post holder to be perceptive and responsive to change with significant cognitive abilities required to interpret information, behaviours and patient cognitions to arrive at accurate conclusions in order to formulate patient presentations and test hypotheses  Ability to concentrate for prolonged periods whilst producing written reports including reports for Tribunals and court reports.  Prolonged periods of concentration required during supervision, training, appraisal and teaching  Concentration required for driving Emotional Demands  Frequent exposure to highly distressing or highly emotional circumstances, working with young people, families/carers who present with a range of mental health problems having to ensure robust risk assessment is carried out to maintain their health, safety and welfare. This can present exceptional challenges both in working with the patients and their carers, the ability to discuss highly sensitive issues which may include bereavement, sexual abuse, trauma etc. and in managing personal emotional responses to the work.  Ability to identify, provide and promote appropriate means of support to colleagues exposed to highly distressing situations and severely challenging behaviours.  Provision of accurate risk assessment and subsequent risk management plans  Escorting of, and intervention with patients across hospital and community settings, including patients detained under Mental Health Legislation or those presenting with difficult behaviours as required  Working alone/ travelling alone, frequently on a daily basis

 Making highly complex decisions around appropriate care pathways for young people with complex mental health problems and/or initiating comprehensive management plans to guide other staff including medical staff in managing individuals/groups of patients.

 Responsible for ensuring effective communications across several disciplines / agencies involved in patient’s care.

 Responsible for managing a clinical caseload where challenging behaviour can occur due to the complex nature of patient/client group and where there is a high level of responsibility for accurate risk assessment and risk management plans.

 Managing competing demands on time, working within a busy outpatient environment whilst also providing advice and consultancy to staff in other areas of service delivery.

 Responsible for providing information to patients/clients and carers on sensitive clinical and legal issues e.g. legal/detention information.

 Responsible for managing the educational environment, including teaching and supervising junior staff, nurse learners and pre-registration nursing students whilst working within a busy clinical environment. Involved in the teaching and supervision of evidence based practice to other disciplines including under graduate and post graduate learners and medical staff

 Maintaining professional identity within a multi-professional psychological therapies service

 May work within inpatient areas and be exposed to highly unpleasant substances in that environment.

12. KNOWLEDGE, TRAINING AND/OR EXPERIENCE REQUIRED TO DO THE JOB

 Registered Mental Nurse, AHP, Social Worker or (child trained CAAP) with 5 years post registration experience.

 Two years recent practice with children and families with mental health problems or learning disability

APPLICATIONS SHOULD BE RETURNED TO [email protected] Or RECRUITMENT, HR SERVICE CENTRE, LAW HOUSE, AIRDRIE ROAD, CARLUKE ML8 5EP IMMUNISATION In order to protect patients, healthcare workers who carry out Exposure REQUIREMENTS, Prone Procedures (EPPs) as part of their role, must provide documented HEPATITIS B evidence that they do not have a blood borne virus that they may pass AND on to patients whilst carrying out EPP work (i.e. that they are not HEPATITIS C infectious for hepatitis B, hepatitis C and HIV). Blood test results must STATUS be from “identified validated samples” (IVS) and be documented as such. In some cases, it is possible for healthcare workers who have a blood borne virus, to carry out a range of EPP work. This will be assessed and monitored by an Occupational Health Consultant and the healthcare workers’ treating physician.

Healthcare workers who perform EPPs have a professional, ethical and legal duty to protect their patients and inform occupational health if they know (or suspect) that they are infected with a blood borne virus (hepatitis B, hepatitis C or HIV) now or at any time in the future. Further guidance is given by the relevant Regulatory Bodies.

DISCLOSURE OF Dependant on the nature of the post we will need to conduct a CRIMINAL criminal CONVICTIONS OF record check. This could be a basic or standard level or a PVG for NHS STAFF regulated work.

In accordance with the Protection of Vulnerable Groups (Scotland) Act 2007, individuals entering regulated work for the first time will be required to join the scheme.

A Scheme Record Update will be obtained for individuals who are registered through the PVG Scheme as already doing regulated work with children or adults.

Please note: You do not have to do anything about PVG Scheme Membership at application stage. Candidates selected for appointment will be advised on how to become a PVG Scheme member prior to taking up the position.

No approach will be made without written permission of the successful applicant, who will be asked to sign and complete the relevant Disclosure Scotland PVG Scheme application, giving authorisation for the check to be undertaken.

The cost of the PVG Scheme membership to the individual is a one off payment of £59. NHS Lanarkshire reserves the right, following consultation with you, to recover from you any payment made in respect of PVG Membership
