STATEMENT OF WORK

1.1 GENERAL BACKGROUND INFORMATION A. Recovery-oriented cognitive therapy is evidenced based and has been proven successful in promoting recovery within inpatient and outpatient settings with individuals who have not responded significantly to previous treatment approaches for serious and persistent behavioral health disorders. Increased successful community integration upon discharge, decreased lengths of stay and decreased re-hospitalizations are among the reported positive outcomes of Recovery Oriented Cognitive Therapy.

B. The proposed Recovery Oriented Cognitive Therapy program will be implemented at Norristown State Hospital to achieve its overall goal of engagement of individuals being served who have not responded to previous treatment interventions by assisting individuals to identify their personal life goals; and resulting in increased community engagement and personal health progress towards recovery both in the civil and forensic programs of Norristown.

C. The Contractor will utilize a Train the Trainer approach to provide training to staff regarding recovery-oriented cognitive therapy and will provide ongoing consultation to staff as requested by Norristown State Hospital.

D. The Contractor will conduct assessments and consultations of unit milieu, primarily, but not exclusively, during day shifts, as well as challenging case consultations with the staff within specifically identified Civil and Forensic units, which will be completed with the resultant development of a recovery supporting community to include therapeutic community, group therapy and individual therapy.

E. The Contractor must use a train the trainer approach which allows Norristown State Hospital to provide continuity of care regarding Recovery Oriented Cognitive Therapy for providers who will be serving the individual following discharge from Norristown State Hospital.

F. The Contractor will provide both challenging case and milieu consultations weekly, in two (2) forensic units and one (1) civil unit. At the direction of Norristown State Hospital, in consultation with the Contractor, the Contractor will progress to having completed challenging case and milieu consultations anticipated to begin September 15, 2015 through June 30, 2016 with a one year term renewal option.

G. At the direction of the Program Coordinator, the Contractor will provide for any emergent challenging case or milieu consultations in place of or in addition to the normal weekly consultations. 1.2 NATURE AND SCOPE OF THE PROJECT The Department of Human Services (DHS) is issuing this Tier II RFQ for Recovery Oriented Cognitive Therapy Training and consultation with an anticipated effective date of September 1, 2015 through June 30, 2016 with a one year term renewal option. The awarded Contractor, once chosen, may not begin services until a valid contract has been approved and the vendor is in possession of a valid purchase order.

1.3 GENERAL REQUIREMENTS A. The Contractor must identify the use of any subcontractors in its proposal. During the contract term, the Contractor must comply with the terms and conditions applying to the use of subcontractors, including obtaining Norristown State Hospital’s prior written approval for the use of its subcontractors. Norristown State Hospital reserves the ability to reject any subcontractor.

B. Contractor is to comply with safety standards and policies at Norristown State Hospital as well as participate in orientation which will include, but not be limited to relevant hospital and Department of Human Services policies, crisis intervention overview and any other policies determined appropriate for the completion of the purchase order term. Orientation will take approximately 3 hours.

C. Contractor is responsible for completing verification of onsite hours and will submit hours and verification in the manner and frequency identified by Norristown State Hospital. Payment will be based for work conducted onsite hours only.

D. Contractor will be responsible for all tasks/deliverables as outlined in the Purchase Order.

1.4 INFORMATION HANDLING A. The nature of this project requires the handling of sensitive and confidential information. The vendor must uphold the policies of the Commonwealth and Norristown State Hospital regarding the confidentiality of all information belonging to the Commonwealth, DHS and all protected health information. The Contractor agrees that any breach of this provision may, at the discretion of DHS, result in immediate termination of the contract and may result in possible legal action.

B. All staff assigned to the project by the Contractor will be required to sign a confidentiality statement. All client information which may be accessed or viewed is to be treated as confidential. 1.7 QUALIFICATIONS A. Demonstrated performance in the provision of train-the-trainer training in support of the recovery of persons with serious and persistent mental illness with verification of credentials in Recovery Oriented Cognitive Therapy.

B. Have provided training, assessment and consultation, with forensic and civil patients, for a minimum of five state psychiatric hospitals, or comparable settings with civil and forensic programs, regarding Recovery Oriented Cognitive Therapy.

C. Have documented improved outcomes within the Substance Abuse and Mental Health Services Administration (SAMHSA) defined Recovery Dimensions with individuals served at the facility/program where the training, assessment and consultation were provided.

D. Has provided assessment and consultation with forensic and civil patient populations at a minimum of five state psychiatric hospitals, or comparable settings, regarding evaluation of the treatment milieu including staff communication and interaction with individuals receiving treatment. The documentation should show actions taken to help staff refine or develop skills that promote recovery and overcome obstacles to recovery for individuals who have not responded with previous treatment approaches.

E. The minimum requirement for each trainer must be a Master’s Degree in Behavior Sciences such as Psychology, Social Work, Counseling, etc. Trainers must also have a minimum of 2 years’ experience in training of Recovery Oriented Cognitive Therapy.

The awarded Contractor will be responsible for successful completion of the following deliverables/tasks:

A. Forensic Challenging Case Consultations – Norristown State Hospital, will identify, in consultation with the contractor, a minimum of 2 forensic units to provide challenging case consultations as requested, weekly. Each unit has a maximum of 26 individuals per unit and anyone served on each unit may present a need for consultation.

B. Forensic Milieu Consultation – Complete a milieu consultation in the identified units, weekly.

C. Civil Challenging Case Consultations – Norristown State Hospital will identify, in consultation with the Contractor, a minimum of 1 civil unit to provide challenging case consultations as requested, weekly. Each unit has a maximum of 30 individuals per unit and anyone served on each unit may present a need for consultation.

D. Civil Milieu Consultation – Complete a milieu consultation in the identified units, weekly.

E. Quarterly Report, Consultations - The selected contractor will submit to the facility’s Contract Monitor and Project Coordinator, specific quarterly reports that list trainings and consultations, completed and planned events that have not yet occurred, and a summary of issues discussed, as well as actions to be implemented to address identified issues. The initial start of the Quarter will be established by the facility. Quarterly reports are due within 30 days upon completion of the quarter.

F. Quarterly Report, Training Effectiveness - The selected contractor will submit a quarterly report that assesses the effectiveness of the training by eliciting feedback from staff and individuals served regarding consultations and assessments. G. Norristown State Hospital Orientation Training – Contractor staff orientation training of up to 6 contractor employees. Orientation training of any additional Contractor employees will be at the expense of the Contractor.
