To reserve a stall for the Old Fort Days Futurity and/or Super Derby, please complete and return this form promptly with payment to: Old Fort Days Barrel Futurity/Super Derby, P.O. Box 4145, Fort Smith, Arkansas 72914.

Stalls will be assigned by the earliest post marked date with payment. Stall reservations must be received no later than March 1st. Due to limited stall availability, permanent stalls will be limited to futurity or derby horses only. Once the permanent stall barns are filled to capacity, horses will then be assigned to tent stalls. For your conve- nience, you can reserve a tack stall; however, all tack stalls are limited exclusively to tent stall areas.

If you have several horses that you prefer to be stalled together, please indicate this information on the Stall Reser- vation Form and mail reservations for all horses together at the same time.
