Paperwork:

You will need an Incyte pathology thyroid worksheet, specimen manifest, and cytology\histology request form. All of these forms are located in the file cabinet in the insurance forms drawer. There are examples of all of the forms filled out so that you will know what you need to fill in. Once the patient arrives you will need to call Chris or whomever is working at the front desk at 37111 and ask for an insurance form on the patient. Also you will need to print Film ID stickers to label the patient samples. If the FNA is a single nodule you will need to print 6 stickers and if it is a double you will need 10.

Set up:

For sterile set up (the tray) there is a list of everything you will need in the Invasive binder.

For the slides:

For each nodule you will need 6 slides. (The slides and the slide holders are located above the assistant desk.) On each slide, in pencil, you will need to write the patients name and “AIR” on the white part of the slide. Each holder will get a film ID label on the outside. You will need one bottle of cytolyte for each nodule that gets a label as well. On your slide tray you will need a 10cc syringe filled with cytolyte fluid to use during the biopsy. Once the procedure begins the Rad will hand you the biopsy needle and you will put a drop of the sample on one slide and place another slide face down on top of the slide and prepare your slide. You will then take the cytolyte syringe and attach it to the needle and flush the needle into the cytolyte. You will do this for each pass (There will be three for each nodule. If the rad does more than 3 passes you can just flush the needle into the cytolyte. You don’t need to do more than 3 slides.)

After the Biopsy:

After the biopsy is finished you tape the slide holders closed and place the slides and cytolyte into the small PAML bag. If there is more than one biopsy scheduled that day, each one gets its own small bag (with a label on the outside). In the outside pocket of that bag you need to put the insurance form, thyroid worksheet, and request form. All of the small bags then get put into a large bag. All of the patients for the same day can get sent in at one time. Each name needs to be written on the specimen manifest and it is placed in the outside pocket on the big bag. The last label gets placed in the PAML JOBS binder. To call for a pick up dial 927-6253 and then press 1. Ask for a job number before you hang up and write it on the patient labels in the binder. The samples will then get taken to the front desk upstairs for pick up. There is a box at the front desk that they get placed in.

Printing labels:

Click on the box next to the patient name in centricity. Click Tracking, then click patient. Click box next to the exam that you are performing and hit print in the upper right hand side of the screen. Use the drop down to select Film Label. Change the printer to SCCO and hit tab; select downstairs label printer. Change the number to how many you need. Hit print. The printer is sitting on John’s desk.
