Community Outreach Health Educator

JOB SUMMARY: Facilitate and coordinate community Health Center outreach events to assist in attracting patients to the centers within Newark Public Schools and the community. Participate in events and evaluate results to determine best and most efficient use of resources. Follow up on cold and warm leads to create exposure within the surrounding community as directed and create a strong community presence. Host health education groups, public health class room chats and community health education summits; that discuss asthma, weight, diabetes and obesity prevention and management. Plan and facilitate our outreach strategies and activities resulting in measurable outcomes.

POSITION REPORTS TO: Director of Newark School Based Health Centers HOURS: 40 hour minimum; Evenings and Weekends as needed.

ESSENTIAL FUNCTIONS: 1. Facilitate comprehensive outreach and community educational program teachings about various public health topics from a broad social context or specific to each health center school. 2. Works to achieve program-based curriculum content standards and program goals by promoting active learning and skills development of participants. 3. Develops outreach and instructional materials and provides opportunities for individualized and small group instruction related to community education and outreach activities as directed. 4. Maintains records of community outreach and educational progress. 5. Facilitate technological social media health education strategies with School Based Director and Marketing Director. 6. Works with program staff to identify and connect relevant services, resources, and interventions that addresses participants' needs, and cooperates with other professional staff in assessing and resolving social, educational, medical, and/or other related challenges affecting the participant. 7. Works with program staff to develop and implement communication strategy with the parents/caregivers of participants. 8. Assists School Based Director to develop and implement wide-ranging community outreach activities as directed. 9. Maintains professional competence and continuous improvement through in-service education and other professional growth activities. 10. Makes effective use of community resources essential in achieving community outreach and educational activities. 11. Works with program staff to help participants and families identify and provide access to resources to support them in addressing key social, emotional, and personal factors effecting their health. 12. Advocate for and with participants and families for services and systems that are fair, responsive, and accountable to the families served. 13. Mobilize formal and informal resources to support family and youth development. 14. Work closely with other staff, local providers, and other stakeholders to assess interests and needs of youth. 15. Develop health education coalitions as directed along with key partners in the community. 16. Manage partnership relationships professionally and as directed. 17. Responsible for ensuring participant tracking and program utilization forms are complete on a timely and accurate basis. 18. Complies with overall reporting and administrative requirements as established by the School Based Director. 19. Other duties and responsibilities as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES: . Bi-lingual in English and French, Creole a plus . New Jersey Teacher Certification in good standing . Maintain a positive and professional customer service attitude . Demonstrated ability to adapt to changes efficiently . Ability to follow instructions, given either orally or in writing . Sensitive to interagency collaboration dynamics . Demonstrate and communicate effective follow-through with all tasks . Maintain high level of confidentially on patient information . Ability to complete assignment in a timely manner . Working knowledge of JRMC/Newark Public School policies and procedures. . Able to work as a team player. . NJ Driver’s License and reliable transportation

TRAINING AND EXPERIENCE: Candidates should possess an undergraduate degree in the areas of health, education, nursing, sociology and/or similar field. A relevant New Jersey Teacher certification in good standing would be preferred. BA level education and/or background in urban public health preferred.
