Staff Evaluation Forms: Explanation

The annual staff evaluation forms that follow are examples of how an evaluation should reflect the job description. These were written using the Job Description for Preschool Teacher provided in Chapter 4. The “job description” column references the item number(s) that relate to the indicator standard. The “X” indicates the level of competence in that area. Jane Anyone received a rating of “Very Good” on the first standard. The evaluation ends with written a Professional Development Plan derived from the evaluation indicators.

The second evaluation format is similar but, rather than using a check box, this form permits the supervisor to describe the employee behaviors that led to the ratings given. Again, it is followed by a Professional Development Plan.

The staff rating can be used to determine bonus or raise fairly with the director having a documented reason for giving higher performing staff a higher raise.
