All sections must be completed.  Course Description: Provide a general description of the course being presented for accreditation.

 Scope: Identify who the course is intended for, jurisdictions (i.e. Edmonton, Calgary) and delivery methods.

 Program Length: Indicate full length of course.

 Course Development: Identify the sources for course content.

 Training Material Source: Provide list of sources used to compile training materials.

 Course Content: Attach expanded table of contents, course outline with allotted training time per section and participant’s manual.  Course outline attached  Participant’s Manual attached

 Instructor Certification Standards: List courses, certifications required by all instructors, including basic first aid training.  Instructor accreditation applications attached (if applicable)

 Instructor Student Ratio: Indicate the ratio of instructors to students.

 Candidate Prerequisites: List any prerequisites that must be met by students prior to entering this course.  No prerequisites required

 Resources/Equipment: Describe the facilities where the course will be taught (i.e. corporate branch locations, client’s facilities). For corporate in-house, attach a list of branch offices. List all resources and equipment used.

 Testing Process: Explain how and when participants are tested throughout the training process and the standard for determining if participants are ready for the final exam.

 Acknowledgement of Pass: Provide sample of letters or certificates issued to participants in addition to the official Training Certificate issued by the Ministry.  Sample documents attached

 Record Keeping: Describe how long participant records are kept and where.

 Proof of Liability Insurance: Must demonstrate minimum $1 million liability insurance.  Insurance documents attached

Part 4 – Acknowledgements and Declaration I agree that I will follow the Security Services and Investigators Act Training Accreditation Policy, as amended from time to time by the Registrar.
