State the chief objective of this position in a brief statement: The chief objective of the Program Coordinator is to be responsible for the provision of expansion and oversight of Jerolin Management Service’s community based community Intellectually Disabled Waiver programs. Duties include monitoring and coordinating staff schedules and training of community Intellectually Disabled Waiver program staff. The Program Coordinator will work cooperatively in coordinating service delivery with other agencies, departments, etc., in response to the changing needs of the individual.

5% ADMINISTRATIVE Part of the team involved in screening, interviewing and hiring individuals to work in the various programs. Responsible for implementing disciplinary actions, performance evaluations and participating in the termination process of employees who are not performing to the standards of their position description and/or not complying with company policies. Also responsible for completing staff work schedules, if applicable and review of assigned staff timesheets. Assist the Human Resource Associate in scheduling of medication administration, behavior management and CPR/First Aide and other trainings for staff

PROGRAM OVERSIGHT/STAFF SUPERVISION

20% *Provide supervision and oversight to the program.

Provide training to Associate Coordinators and Direct Support Professionals related to providing direct services to individuals and documentation.

*Supervision staff in conducting a program of developmental training in the following areas: Daily Skills, Social/Leisure Skills, Community Living Skills, Health/Safety, Academic Skills, Behavioral Management.

Supervision of individual’s medication and scheduling medication administration training for all staff.

Recommends to Executive Director changes or additions to program activities, including client services, staff development, and facility and equipment needs.

Maintains a safe and orderly environment for individuals and staff by addressing potential health and safety issues.

QUALITY MANAGEMENT/CONSUMER RECORDKEEPING 10% Demonstrate the ability to document effectively, monitor and review the

individual’s records.

Complies with all established legislation, mandated program standards, and Jerolin Management Services, LLC requirements regarding the individual’s services and documentation of services.

Completes, submits and make corrections on monthly record reviews for all charts.

SERVICE COORDINATION 15%

Consistently exercise discretion and good judgment relate to the individual’s services and crisis situations.

Complete intake activities for individuals being admitted to the company and enrolled in a program.

Complete functional assessments to identify the strengths and needs of individuals who have various sensory impairments, mental and physical limitations in an effort to develop an Individualized Service Plan.

Collaborate, communicate and participate in service coordination with case managers, programs, family members, and other agencies to ensure appropriate and supportive services delivery.

Schedules and coordinates the timely provision of ancillary services as needed.

20% PLAN FOR SUPPORTS:

Develop individuals Plan for Support (PFS) based on results of assessment tools utilized to determine skill-building and assist needs.

Monitors outcomes, behavioral interventions, and the individual’s progress and preference according to information identified on the individual’s Plan for Support.

Complete person centered review reports and annual progress reports utilizing the individual’s support notes, data collections and case consultation with staff.

Provide training to Associate Coordinator in developing ISPs, assessments and completing person centered review reports (PCR) and sign completed plans and PCRs validating approval.

10% COMMITEES MEETINGS: Conducts, attends and participates on committees and meetings for the purpose of information exchange, services coordination and quality assurance. Conducts scheduled group meetings and educational trainings with staff, as needed. 5% TRANSPORTATION ASSISTANCE:

Provides and arranges individual’s transportation needs.

10% STAFF DEVELOPMENT: Participate in all required training activities, seminars, and conferences to enhance skills and understanding of service delivery changes.

Participate annually in the following trainings: Confidentiality/HIPPA, Human Rights, CPR/First Aid, Behavior Management, Risk Management-Exposure control Plan, Emergency Preparedness.

Medication Training –one time upon hire and tri-annually thereafter.

Participate in DSP Orientation Training or DSP Orientation Training for Supervisors sponsored by the Department of Behavioral Health and Developmental Services upon hire and prior to working directly with any individuals

Participate in provider and documentation held by the agency. General 10% documentation upon hire and as needed thereafter.

REPORT SUBMISSIONS: Completes and submits all programmatic, financial, audit reports and forms, while meeting identified deadlines. Not Rated OTHER RESPONSIBILITIES – (not measurable- occurs ongoing during program hours)

*Assures the smooth operation of Day Support program.

Monitors and recommends when supplies are needed.

Employee will conduct self in compliance with laws, regulations, and agency policies and procedures.

Perform other related duties as required.

B. Special licenses, registration or certification: Meet QIDP and QMHP standards or equivalent experience. Valid driver’s license with good driving record. CPR/FA, Medication Administration, and Behavior Management Trainings.

Successful completion of criminal history background and central registry investigations.

Education or training (cite major area of study): A minimum of a Bachelor’s degree Human Services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling, or other degree and/or experience deemed equivalent to those described) from an accredited college.

D. Level and type of previous work experience: Minimum of one year of experience (consecutively or concurrently) providing direct services to persons who are intellectually disabled or have other developmental disabilities and clinical experience providing direct services to persons with a diagnosis of and mental illness preferred. One year of supervisory experience, preferred.
