Student Aid Form Filing Instructions

To ensure a smooth processing of your application and avoid unnecessary delays, it is imperative that you complete and submit all applicable parts of the application and submit all required documents online.

Please be aware that local Jewish programs and schools use a shared application. Only one application needs to be completed when applying to any of the programs and schools listed on the School/Agency Code List. Your information will be released by PSAS only to the organizations you have selected.

Current tuition assistance families: please complete and submit your online application to PSAS no later than December 1, 2014. The School will advise families with timely applications of their tuition assistance decisions before the end of February.

Prospective tuition assistance families: please complete and submit your online application to PSAS no later than January 15, 2015. The School will advise new families with timely applications of their tuition assistance decisions in the beginning of March. Please be aware that decisions are released only after a student has been accepted for admission to the school.

In order to process your application, please upload or send the following documents to PSAS:

 Completed application (all relevant questions must be completed).

 Copy of 2013 Federal Income Tax Return with all supporting schedules. Business owners must include Schedule K-1, Forms 1065 and/or 1120S.

 Copies of all 2013 W-2 forms, 1099/1099R or 1098 forms.

 Copies of two most recent paystubs.

 Social Services Year End Statement (AFDC, ADC, TANF) and/or Food Stamp Year End Statement and/or your Social Security Year End Benefits Statement(s) for ALL adults living in the household receiving benefits in 2013.

 Copies of your 2014 W2 forms when they become available in January 2015. Please login to www.my.psas.org to upload additional documents.

 The application fee is $27.00 and payment is required when you submit your application.

Incomplete applications, including those without income tax documentation, will result in your application being placed on hold. To check the status of your application, go to www.my.psas.org, to login.
