Transforming STEM Learning (TSL) Research Study

Dear Parent or Guardian,

In order to learn more about students’ experiences at school, Charlotte-Mecklenburg Schools is participating in a research study called the School Based Mental Health (SBMH) project sponsored by the National Institute of Justice (NIJ) and conducted in collaboration with RTI International, a not-for-profit research organization in Durham, NC. As part of the study, our school will participate in the School Safety and Climate survey during the 2016-17, 2017-18, and 2018-19 school years. Your child’s classroom is one of about 6 classrooms in your child’s school that has been randomly selected to complete the survey this fall (2016). The School Safety and Climate survey is an anonymous paper-and-pencil survey that asks about students’ feelings about overall school climate and their safety while at school. Other questions ask about whether students have participated in or been the victim of bullying. The School Safety and Climate survey will help district and school leaders more effectively serve students.

The School Safety and Climate survey will be administered at school by RTI International and will take about 30 minutes. Overall results will be shared with the school leadership team, though your child’s answers will not be identifiable. The survey will be anonymous. No personally identifying information will be collected, and student names will not appear on the completed survey or in any reports.

Student participation in the survey is completely voluntary; students can choose not to participate or to skip any question(s). They can quit at any time without penalty. Completing this survey will pose little or no risk to your child, though there is a slight chance that your child may want to talk with someone after taking the survey. Students will get information about school staff they can talk to and a list of Hotline numbers they can call if they would like to discuss their feelings and experiences.

Students will be asked to participate in the School Safety and Climate Survey this fall (2016). Your child might be randomly selected to participate again this spring and/or in following years. You may withdraw your consent at any time by contacting the school principal. You can also decline your child’s participation in future surveys by signing the consent form that will be sent home prior to the survey each spring. No action will be taken against the school, you, or your child, if your child does not participate. If you would like your child to participate in the survey in 2016, you are not required to do anything. If you prefer that your child NOT participate in the survey in 2016, please sign below and return this form within 3 days.

Para poder aprender más sobre las experiencias de los estudiantes en la escuela, las escuelas de Charlotte-Mecklenburg están participando en un estudio llamado Estudio de Salud Mental en las Escuelas (SBMH) patrocinado por el Instituto Nacional de Justicia (NIJ), en colaboración con RTI International, una organización no lucrativa que realiza estudios científicos en Durham, NC. Como parte del estudio, nuestra escuela participará en la Encuesta sobre Seguridad Escolar y el Ambiente Estudiantil durante los años escolares 2016-17, 2017-18, y 2018-19. El salón de clase de su niño(a) es uno de 6 salones de clase en esta escuela que fue seleccionado al azar para completar la encuesta durante el otoño (2016). La Encuesta sobre Seguridad Escolar y el Ambiente Estudiantil es una encuesta anónima, en papel y con un lápiz, se le pregunta a los estudiantes qué piensan sobre el ambiente escolar general y su seguridad cuando están en la escuela. Otras preguntas son acerca de la participación de los estudiantes en el acoso escolar o «bullying», o si han sido víctimas de esto. La Encuesta sobre Seguridad Escolar y el Ambiente Estudiantil ayudará a los líderes del distrito y de la escuela a servir a los estudiantes de manera más efectiva.

La Encuesta sobre Seguridad Escolar y el Ambiente Estudiantil será administrada en la escuela por RTI International y tomará aproximadamente 30 minutos. Los resultados generales se compartirán con el equipo directivo de la escuela, aunque las respuestas de su niño(a) no se podrán identificar. La encuesta será anónima. Ninguna información de identidad será recopilada y los nombres de los estudiantes no aparecerán en la encuesta que completen, ni tampoco en ninguno de los reportes.

La participación del estudiante es totalmente voluntaria, los estudiantes pueden decidir no participar o no contestar cualquier pregunta. Ellos pueden detener su participación en cualquier momento sin ninguna consecuencia. Completar esta encuesta tiene poco o nada de riesgo para su niño(a), pero hay una pequeña posibilidad de que su niño(a) quiera hablar con alguien después de realizar la encuesta. Los estudiantes obtendrán información sobre el personal de la escuela con el que puedan hablar y una lista de números de líneas telefónicas de ayuda a los que pueden llamar si desean hablar sobre sus sentimientos y experiencias.

A los estudiantes se les pedirá que participen en la Encuesta sobre Seguridad Escolar y el Ambiente Estudiantil en el otoño (2016). Su niño(a) puede ser seleccionado(a) al azar para volver a participar en la primavera y/o en los años siguientes. Usted puede retirar su consentimiento comunicándose con el director (principal) de la escuela. También se puede negar a la participación de su niño(a) en encuestas futuras, firmando el formulario de consentimiento que se enviará a su casa antes de la encuesta cada primavera. No se tomará ninguna acción contra la escuela, usted o su niño(a) si no participa su niño(a). Si tiene preguntas o preocupaciones sobre el estudio, por favor llame a la Dra. Si desea que su niño(a) participe en la encuesta en el 2016, no necesita hacer nada. Si usted prefiere que su niño(a) NO participe en la encuesta en el 2016, por favor firme abajo y regrese el formulario en los siguientes 3 días.
