Applications are invited for two full time Consultants in Paediatric Anaesthesia based at the new Royal Hospital for Children.

Post 1: 10PA contract with responsibilities to general theatres. The ‘on call’ commitment for this post is 1 in 10, providing cover for emergencies (excluding cardiac) on the RHC site.

Post 2: 10PA contract with responsibilities to general and cardiac theatres. The ‘on call’ commitment for this post is 1 in 6, providing cover for cardiac surgical and cardiology emergencies on the RHC site.

The successful candidates will work along side 17 Consultant Anaesthetists. The department also includes one advanced paediatric anaesthesia trainee, four post-CCT Fellows and specialty trainees rotating from the West of Scotland Training Scheme.

Applicants must have full GMC Registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training. Significant previous paediatric anaesthetic experience is expected.

Applications are invited for two whole time Consultant posts in Paediatric Anaesthesia at the Royal Hospital for Children, Glasgow.

Post 1 will have sessions in the emergency theatre and commitment to general on call. Post 2 will contribute to the cardiac service, including on call. Opportunity for the development of an additional special interest will be encouraged within the service.

The majority of acute children’s services in NHS Greater Glasgow and Clyde are provided from the Royal Hospital for Children (RHC) based on the South Glasgow Hospitals campus with the Queen Elizabeth University Hospital (including Maternity services) in Govan. A paediatric ward and neonatal unit in the Royal Alexandria Hospital supports acute non-surgical care for children and young people and neonates in the Clyde catchment area of the NHS Board. Neonatal intensive care is provided at the Southern General Maternity Hospital, Princess Royal Maternity and Royal Alexandria Hospital.

The Royal Hospital for Children

RHC is the largest paediatric teaching hospital in Scotland with 256 beds over five floors. It provides care for children resident within the Greater Glasgow area, acts as a tertiary referral centre for children from the West of Scotland, and in a range of subspecialties, including invasive paediatric cardiac, nephrology and renal transplantation services, for children from the whole of Scotland.

All paediatric medical and surgical subspecialties are provided, including general medical paediatrics, respiratory, endocrinology, gastroenterology and nutrition, haemato-oncology (bone marrow transplant service), neurology, neurosurgery, cranio-facial and maxillofacial surgery, dental surgery, nephrology, cardiology, immunology and infectious disease, dermatology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, orthopaedic surgery, general paediatric and neonatal surgery, paediatric urology and renal transplant surgery.

There are nine full operating theatres, and two interventional theatres with mono and biplanar screening. Four of these theatres are fully integrated for minimally invasive work, a number have laminar flow air hoods for ultra clean surgery, and the neurosurgical theatre is equipped for stereotactic surgery. Adjacent to the theatre complex are the day surgery and 23 hour unit and the pre-assessment team.

The 22 bedded critical careunit is adjacent to the operating theatres. A link bridge provides access to the neonatal ICU in the maternity hospital.

The paediatric radiology department has a full suite of state of the art scanners and a nuclear medicine department. There are also cardiac & respiratory physiology departments on site. There is a large outpatient department and a medical day care unit. The emergency department is adjacent to the medical assessment unit and to the adult hospital’s emergency department.

A pan Greater Glasgow Child Protection Unit has been developed on site. There is also a Paediatric and Family Psychiatry service at the RHC.

Laboratory services to the RHC are provided from the South Glasgow University Hospitals campus Laboratory Building. National Service Contracts

Southern General Maternity

In 2010 the new neonatal unit opened above the labour ward with projected delivery numbers for the hospital of 5,900 births per year. After the new RHC opened, neonatal services previously provided within the old children’s hospital moved to the maternity hospital’s neonatal unit. There is a link bridge connecting the neonatal unit to the RHC at the same level as the paediatric critical care unit and operating theatres. The Ian Donald Fetal Medicine unit is on site so a significant workload for neonatology involves delivery of infants with known congenital anomalies. There is physical space for 34 intensive care and high dependency cots and 26 special care cots.

Princess Royal Maternity (PRM)

The PRM is a large maternity unit, situated about 1 mile east of the centre of Glasgow. It is on the campus of Glasgow Royal Infirmary, which provides services for the population of the East and North of Glasgow. It is a purpose built 120 bed maternity hospital, with a capacity for 6,800 deliveries per annum. There are 10 intensive care cots and 23 special care cots and a newly opened 4 bed transitional care. Facilities in intensive care include the latest means of respiratory support and monitoring. All neonatal intensive care can be provided, except for those infants who require neonatal surgery and/or extra-corporeal life support services (ECMO). A telemedicine link with the Royal Hospital for Children has been long established allowing live consultation and the transmission of ultrasound images. The University Chair of Obstetrics and Maternal Medicine is based on site and there is an active research philosophy in the unit. 2. Management Structures

3. Anaesthesia, Surgery, Critical Care and Neonatology Services

Anaesthesia, critical care and surgical services are a key component of integrated hospital paediatric and neonatology services within the Women and Children’s Directorate (of the Acute Operating Division, NHS Greater Glasgow and Clyde). Dr Pam Cupples is Clinical Director for Anaesthesia and Surgery. Dr Morag Campbell is Clinical Director for Paediatric Critical Care, Neonatology and Cardiac Services. These roles are supported by link clinicians from specialist areas. Dr Anne Goldie is link for Anaesthesia and for Cardiac Anaesthesia. This structure of clinical leadership is mirrored within medical services.

The Anaesthetic Department

The department is situated on the second floor of the new Children’s Hospital, close to the operating theatres providing offices and personal PCs for all Consultants (each office shared usually by three consultants), an office for the departmental PA, and a trainee workstation/staff base room with data projection. Seminar room space is shared throughout the entire hospital campus.

Each year, we deliver over 10,000 anaesthetics, covering all the major sub- specialties of paediatric anaesthesia. These include anaesthesia for General and Neonatal surgery, ENT and Major Airway surgery, Cardiac, Thoracic, Orthopaedic, Trauma, Dental, Craniofacial and Plastic (including Cleft Lip and Palate) Surgery, Neurosurgery, Oncology, Urology and Renal Transplantation, Gastroenterolgy, Diagnostic Imaging and Interventional Radiology, Diagnostic and Interventional Cardiology, Regional Anaesthesia, and Acute and Chronic Pain Management. We also provide anaesthesia for children requiring Radiotherapy at the Beatson Oncology Centre in the north west of the city.

There are 17 consultants in the Anaesthetic Department. Four additional consultants have part-time commitments to the Department providing anaesthesia for elective surgical lists. Ten consultant anaesthetists have sessions in the Emergency theatre and provide on call for general emergencies. Six consultant anaesthetists provide anaesthesia for elective and emergency cardiac surgery and for some elective and all emergency cardiology. The cardiac surgical programme carries out approximately 300 cases annually and covers all surgical procedures except transplantation.

Trainees on the West of Scotland Anaesthesia Rotations attend the Department for 2 month periods at junior and senior level. In addition there is a one year advanced training post in Paediatric Anaesthesia. We also have a up to five post-CCT Fellows who are appointed annually. Theatre Complex

Situated on the first floor, the theatre complex provides state-of-the-art facilities. There are nine theatres and two interventional theatres. Adjacent is part of the diagnostic imaging department with access to CT and MRI scanners and nuclear medicine, all with capability for delivering general anaesthesia. Purpose-built Day Surgery and 23 hour units are situated at one end of the theatre complex.

Pain Services

The Anaesthetic Department runs an Acute Pain Service co-ordinated by two Nurse Specialists in Pain Management (Ms Kay Trower and Ms Margaret McCulloch) and Lead Clinician for Acute Pain, Dr Tony Moores. Nine other Consultant Colleagues contribute to the clinical service.

The Department also provides one of the very few multidisciplinary chronic pain management and symptom control programmes for children within the UK.

Critical Care Services

The 22-bedded PICU at the RHC in Glasgow is the lead centre for the care of critically ill children in Scotland. Each year, 650 – 700 children are admitted, including 80 transferred from other hospitals throughout Scotland. The National Paediatric Critical Care Transport Service is run jointly with Edinburgh’s Royal Hospital for Sick Children. The Unit is one of four Extra Corporeal Life Support centres in the UK.

4. Information on the Consultant Posts in Paediatric Anaesthesia

The posts are for two Consultants in Paediatric Anaesthesia. All applicants must be eligible for full GMC registration and either be already on the Specialist Register or be within 6 months of receiving their CCT (certificate of completion of training) at the time of interview. The applicants should have undertaken at least 12 months’ training in Paediatric Anaesthesia. Additional experience in PICU and formal qualification as a provider for paediatric resuscitation (APLS / PALS / EPLS) are desirable.

Post 1 with sessions in Emergency Theatre and General on call On a 10 PA contract (9 DCC, 1 SPA), the successful candidate will provide cover for 6 theatre sessions per week and 1 DCC for on call. Up to two additional EPAs may be available for anyone wishing to work more than 10 PAs a week.

Post 2 with sessions in Cardiac Theatre and Cardiac on call On a 10 PA contract (9 DCC, 1 SPA), the successful candidate will provide cover for 6 theatre sessions per week and 1 DCC for on call. Up to two additional EPAs may be available for anyone wishing to work more than 10 PAs a week. Applicants should be able to demonstrate an interest in paediatric cardiac anaesthesia. Additional training in paediatric cardiac anaesthesia will be considered for the successful candidate if required. This appointment will allow succession planning for retirement of one of the cardiac anaesthetists during 2016. Sessions in Cardiac Theatre will build up following appointment to allow for a smooth transition. The same will apply for on call, which may not form part of the job plan in the early stages. During this period, DCC allocation for on call will instead be spent in elective cardiac theatre or cardiology.

Interim Job Plans will be agreed between the appointees and the Clinical Director for Anaesthesia & Surgery on behalf of the Chief of Medicine for the Women and Children’s Directorate prior to commencement. The Job Plans will be based on the provisional timetables shown below. A number of changes have been made to existing consultants’ theatre sessions as a result of the move to the new hospital and the numerous changes made to surgical theatre allocations. These changes are continuing, therefore the successful candidates will be expected to show flexibility as their job plans are agreed.

NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

Please note that, in the event of a scheduled list not running (for example if the surgeon is on leave) or if there is a more pressing need for Consultant cover of another list, the post-holder will be expected to work flexibly within any of the fixed clinical sessions. In practice, this provides considerable diversity and ensures that experience is maintained in all aspects of the paediatric anaesthetic service.

In addition, the above job plan is only included as a guidance to potential candidates. Under the terms and conditions of the new Consultant Contract the successful candidate will be able to negotiate their job plan with their Link Clinician / Clinical Director as noted earlier.

Duties of the Post Holder

To provide anaesthesia for children having treatment at the RHSC and occasionally on other sites (including the Beatson Oncology Centre), if agreed with the Link Clinician for Anaesthesia / Clinical Director for Anaesthesia, Critical Care and Surgery.

Alteration in work patterns may occur with further expansion in workload and Consultant numbers.

In the event of cancellation of a scheduled clinical activity, the post-holder would be expected to undertake other Clinical Duties or additional Supporting Professional Activities on site during that time.

The Consultant has a continuing responsibility for the care of patients in his / her charge, and for the proper functioning of his / her Department.

The Consultant will be expected to supervise and teach trainees whilst working in theatre.

The Consultant will undertake any administrative duties associated with the care of his / her patients and the running of his / her Department.

The Department currently holds a monthly business and CPD meeting over half a day for which elective work is cancelled. Consultants are expected to attend this meeting regardless of whether or not they would have normally undertaken clinical work at that time, unless they have approved leave. Maintenance of CPD is essential. If the monthly Departmental meeting occurs when a Consultant would usually expect to undertake elective clinical work AND he / she is scheduled for fewer than 38 hours of clinical work each week (equivalent to 7 half-day theatre lists; plus on call 12 PA Contract) on their agreed job-plan, then the Consultant would be expected to make the clinical time lost available elsewhere within the same or another week.

The Consultant would be expected to participate fully in Appraisal, Risk Management, Audit, Critical Incident Reporting and other Clinical Governance Activities.

The successful applicants will be expected to work with local managers and professional colleagues in the efficient running of services and will share with Consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions, you are expected to observe the Division’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instruction of NHS Greater Glasgow. In particular, where you manage employees of the Division, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary. All medical and dental staff employed by the Division are expected to comply with all Hospital Health and Safety Policies within the Division.

QUALIFICATIONS Registerable medical degree e.g. M.B. Ch.B. Higher degree or other Fellowship of the Royal College of Anaesthetists (FRCA) medical (or equivalent) (e.g. MRCP, MRCPCH) or relevant professional (e.g. Post Graduate Certificate of Medical Education, At the time of taking up appointment, must be either on Masters in Business the Specialist Register or within 6 months of award of Administration) CCT in Anaesthesia qualification

Recognised qualification TRAINING Recognised qualification as a provider of paediatric as instructor in paediatric resuscitation (e.g. APLS, EPLS, PALS or equivalent) resuscitation (e.g. APLS, EPLS, PALS or equivalent)

12 months experience in paediatric anaesthesia at Specialist Registrar or post-CCT (Certificate of Completion of Training) or equivalent grade 3 month’s experience in PICU

AUDIT Evidence of participation in audit activities Evidence of interest in research and academic RESEARCH activities

Presentations of audit or PUBLICATIONS / PRESENTATIONS research at national / international meetings or within peer reviewed journals

Postgraduate distinctions, OTHER prizes, awards or grants

KNOWLEDGE Skills appropriate to a doctor judged competent to have Expertise in managing AND completed Specialist Registrar Training in Anaesthesia difficult airways in SKILLS children Evidence of specific anaesthetics training - including the sub-specialties of paediatric anaesthesia (including for Commitment to infants having specialist surgery) and acute pain continuing personal management medical education

Evidence of an interest in the sub-specialty of paediatric anaesthesia

DISPOSITION Able to relate well with patients and other members of healthcare team

Commitment to multi-disciplinary team working

Evidence of leadership qualities

Evidence of organisational and communication skills

Evidence of appropriate level of confidence i.e. able to work under pressure but understands limitations and when to seek help.

Prepared to act in a supervisory role to junior anaesthetists in training

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

£ 76,761 to £ 103,490 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40.00

SUPERANNUATION New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. Should you choose to "opt out" arrangements can be made to do this via: www.sppa.gov.uk

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.

EXPENSES OF Candidates who are requested to attend an interview will be given CANDIDATES FOR assistance with appropriate travelling expenses. Re-imbursement shall not APPOINTMENT normally be made to employees who withdraw their application or refuse an offer of appointment.

TOBACCO POLICY NHS Greater Glasgow and Clyde operate a No Smoking Policy in all premises and grounds.

This post is considered to be in the category of “Regulated Work” and DISCLOSURE SCOTLAND therefore requires a Disclosure Scotland Protection of Vulnerable Groups Scheme (PVG) Membership which currently costs £59.00. The cost of the PVG Membership will be initially paid by NHS Greater Glasgow and Clyde and will required to be repaid through a payroll deduction mandate from the successful candidate’s first salary.

REHABILITATION OF The rehabilitation of Offenders act 1974 allows people who have been OFFENDERS ACT 1974 convicted of certain criminal offences to regard their convictions as “spent” after the lapse of a period of years. However, due to the nature of work for which you are applying this post is exempt from the provisions of Section 4 of the Rehabilitation of Offenders Act 1974 by virtue of the Rehabilitation of Offenders Act 1974 (Exceptions Orders 1975 and 1986). Therefore, applicants are required to disclose information about convictions which for other purposes are “spent” under the provision of the act in the event of employment, failure to disclose such convictions could result in dismissal or disciplinary action by NHS Greater Glasgow and Clyde.
