No later than one (1) week following your Eagle Board of Review, return the following four (4) documents to the Central Florida Council Eagle Scout Registrar:  This Checklist & Intake form.  Your Original Eagle Scout Rank Application with the date of your Eagle Scout Board of Review and appropriate signatures with Life Purpose Statement, Project Completion Signature Page and Eagle Scout Service Hours Certificate attached.  The Advancement Report for your unit which notes your Unit, District, Name and Eagle Scout Board of Review Date.  The completed National Eagle Scout Association (NESA) Eagle Scout Service Project Information form.  A headshot photo – Please email directly to Traci Shepherd: [email protected] if you have the photo in digital format already. If not, then include with the above documents (will be returned). This photo is used in the Council Recognition Banquet yearbook/program. The banquet is scheduled for June 2016 and will honor the previous year’s Class of Eagle Scouts. Invitations will be sent early 2016. Photo must be received by February 1, 2016.
