[Name of Recipient] [Title] [Company] [Address] [City, State Zip]

Dear [Name of Recipient]:

At CVS Health, we are committed to providing you with meaningful benefit choices. We listened to your needs and are pleased to introduce a company-funded Health Reimbursement Account (HRA) on June 1 for colleagues who are ineligible for a Health Savings Account (HSA). To be eligible for an HSA, you must only be covered by an HSA-compatible health insurance plan, not enrolled in Medicare and not claimed as a dependent on another person's tax return. You also must not have received any Veteran’s Administration health benefits in the last three months.

CVS Health will fund the HRA for you and contribute the same annual amount you’d receive if you were eligible for an HSA. The amount will be based on your salary as of February 28, 2015. Your Tobacco-free Pledge reward will also be deposited to your HRA, if applicable. ConnectYourCare will administer your HRA.

Health Reimbursement Account

Use the HRA to pay for eligible health care expenses like prescriptions, deductibles and much more. It’s easy to use and includes a payment card, online and mobile access and round-the-clock customer service.

Only the company can make contributions to your HRA, and your balance will roll over to the next plan year. However, the HRA balance is not yours to keep if you drop coverage or leave the company.

Important Action Required

To receive the company-funded HRA, you must select the reason you are ineligible for the HSA on myHR.cvs.com during annual enrollment (April 3-24). We will automatically open and fund your account.

Please determine your HSA eligibility before annual enrollment to ensure you make the right benefit choices for yourself and your family. If you have any questions, please contact myHR at 888-694-7287. We look forward to providing you and your family the health care benefits that best meet your needs.

Sincerely,

Lisa G. Bisaccia Executive Vice President Chief Human Resources Officer
