4. SCOPE AND RANGE

General:

. Demands for Diagnostic Imaging are generated by the specific needs of each clinical area. Images must be suitable for diagnosis and archived on the PACS. . Senior Radiographers will be available on a daily basis and provide direct supervision and training when required during core working hours. However, when providing an out of hours service the radiographer may take on the senior responsibility. . A Superintendent or Senior Radiographer will agree an annual personal development plan.

Clinical Areas:

. General Radiography including A/E, GP, In-patients, Out-patients, Dental, Fluoroscopy, Theatre, Mobiles and CT scanning. . Service covers imaging patients from neonatal to geriatric including individuals who have varying degrees of ability and understanding.

Clinical Activity:

. Ranging from 100,000 to 140,000 examinations per annum depending on the site.

Clinical Provision:

. 24 hours / 365 days per annum. There is a requirement to provide an out of hours service to cover nights and weekends for A/E patients, in-patient examinations, CT scanning and fluoroscopy procedures.

Staff Responsibility:

. Participate in the training and induction of new members of staff. . Participate in the training of Student Radiographers. . Direct and supervise the workload of the Radiographic Helpers.

5. MAIN DUTIES/RESPONSIBILITIES

Imaging examinations use highly dangerous radiations. It is the post holders responsibility to minimise radiation dose and risks to patients, staff, relatives, general public and self. This minimises the risk to current and future generations.

Clinical:

. To care for the needs and welfare of every patient. . Act independently in the assessment of referrals for x-ray examinations, taking full responsibility for the authorisation of general x-ray examinations in order to reduce unnecessary ionising radiation exposure of patients in accordance with IR(ME)R 2000. . Verbal and written communications are used daily to query incorrect or unnecessary referrals in order to reduce patient dose or suggest a more suitable examination. . Patients will have injuries or illness that will require the adaptation of the imaging technique, utilisation of developed motivational and persuasive skills to acquire accurate position and reduce mobility to produce an optimal diagnosis with minimal dose. . Operate high pressure Intra-arterial / Intra-venous injector pumps. . To assess patient condition whether that be physically, mentally and/or emotionally prior to and during each examination in order to provide the best possible image. . To clinically evaluate images in order to assess quality, and to determine the need for additional images to further aid patient diagnosis. . Maintain a high level of expertise in the safe operation of x-ray equipment and manage faults effectively. . Provide advice to clinicians on the nature of a diagnostic image either by highlighting abnormalities verbally or using the abnormality detection system to identify possible pathology. . Work as part of a team to ensure effective communication and delivery of care. . Prioritise workload depending on the severity of a patient condition and the direct impact on their management. . Liaise with fellow healthcare workers and referring clinicians to provide a high quality imaging service to patients. . Maintain accurate patient records by input of accurate information to reflect the service provided and meet professional standards. . Work independently when providing an ‘out of hours’ service to patients requiring urgent imaging due to an acute trauma or illness. . To be familiar with and adhere to the Control of Substances Hazardous to Health Regulations, the Health and Safety at Work Act and IR(ME)R 2000. . To undertake general X-ray examinations in satellite departments when required. . Maintain a clean and safe working environment for both patients and staff in accordance with infection control policies.

Managerial:

. Be able to exercise personal responsibility and make decisions in complex and unpredictable circumstances. (E.g. Imaging in A/E and theatre during a multiple trauma situation.) . To be held accountable for all aspects of own work. . Frequently reassess own workload to provide better utilisation of radiographic resources. . Communicate with all staff on any aspect that affects the daily management of the department. . Be actively involved in departmental audit and Quality Assurance to maintain effective Clinical Governance. . Delegate appropriate tasks and supervise Radiographic Helpers and Clinical Support Workers to achieve the desired quality of patient care.

Educational:

. Participate in mandatory training and actively pursue Continuous Professional Development keeping an up to date personal record. . Maintain knowledge of technological advances in methods of diagnostic imaging in order to promote a culture of continuous improvement within the department. . Develop knowledge and understanding within an evidence based framework and transfer to situations encountered in practice. . Be actively involved in the training and assessment of Student Radiographers on clinical placement, providing direct supervision at all times. 6. EQUIPMENT & MACHINERY

A variety of specialised investigative and diagnostic imaging equipment is utilised which comprise of multifunctional controls. The equipment is operator dependent and requires specific skills to achieve images of diagnostic quality. Below is a list of the radiology equipment. Post holders use the majority of the equipment relevant to their department. Radiographic equipment ranges from £30,000 to £1million.

. General purpose x-ray equipment . Dedicated resus x-ray equipment . Digital fluoroscopy units . Tomography units . Isocentric skull units . General dental units . OPT/Ceph units . Multi slice CT scanners . MRI scanners . Dedicated Cardiac Cath Lab . Dedicated digital Angiography Unit . Dedicated Coronary Care pacing unit . High Pressure Injector Pump . Mammography suite . Bone densitometry suite . Mobile x-ray units . Mobile image intensifiers . Kodak Computed Radiography Systems . Darkroom processing equipment . Personal Computers . Manual Handling Aids: Mechanical patient hoists, PAT slides, GLIDE sheets

7. SYSTEMS

The following are used on a daily basis: . Computed Radiography (CR): all images are viewed on remote operator panels prior to being accepted onto the archiving system. This allows for all details to be checked and images to be manipulated to enhance diagnostic quality. . Picture Archiving Communications System (PACS): used in a daily basis to retrieve previous examinations and store current images. Used to copy images onto film for use in theatre or for transfer to other hospitals. . Radiological Information System (RIS): used to input and access patient information relating to radiological examinations in order to review previous procedures and clinical history, record current examination details and reports. . Hospital Information System (Panther): electronic order/communication system for requests and reports. . Patient Management System (PMS): used to track the route of the patient to determine ward, location of case notes and appointments. . Digitiser: enables x-ray films to be scanned and transferred onto the PACS.

8. DECISIONS AND JUDGEMENTS

. Independently evaluate clinical information provided by diverse clinical disciplines to decide whether requested examinations are justified or reject if inappropriate (advise/ discuss with referring doctor if exam is unjustified). . Consider mechanism of injury prior to x-ray procedures and decide upon the appropriate views to obtain the best possible diagnostic outcome. . Assessing and understanding a wide range of clinical information, from a variety of clinical disciplines. . Interpreting clinical information and deciding whether requested examination be justified. . Assessing mental, physical and emotional condition of patient prior to examination, in order to determine the best possible methods to employ, using existing resources, and to minimise risks. During examination, technique should be modified should patient’s condition change. . Assess condition of patient to determine if a reaction is exhibited after contrast has been administered. . After examination ensure patient understands procedure for receiving results, and is well enough to be discharged from the department. . Aware of how quickly controlled and stable situations, can soon become emergency, life threatening conditions, and can respond with speed and accuracy to minimise delays in treatment or diagnosis. . Can recognise abnormal appearances on images and make a decision on whether further imaging is required. Will support this judgement by highlighting abnormalities and discussing radiographic findings with referring clinician if necessary. . Assess images for diagnostic and photographic quality. . Assess ability and competences of students to perform radiographic tasks.

9. COMMUNICATIONS & RELATIONSHIPS

Daily contact with:

Patients:

. Providing complex information by explanation of procedures, listening to the patient’s requirements in order to encourage compliance with the imaging process, e.g. concerns over radiation dose or regulations regarding pregnancy. . Some patients have a barrier to understanding or are unable to communicate, the radiographer must try to allay fears by ensuring that patients have the benefit of informed choice. . Providing and receiving highly complex and sensitive information e.g. dealing with non-accidental injuries in children and dealing with IV drug abusers. . Communication skills are adapted to meet the needs of patients who may be anxious, aggressive or intoxicated, with a variety of mental and physical abilities, in pain or of ethnic origin. The barriers to understanding must be overcome using clear, comprehensive, sympathetic and persuasive skills. . Patients will have injuries or illness that will require the adaptation of the imaging technique and utilisation of developed motivational and persuasive skills to ensure the correct position and to reduce mobility, thus producing a high quality diagnostic image. . When requested, provide factual information regarding the risks associated with an imaging procedure.

Relatives / Carers:

. Provide information using tact and diplomacy in the context of the standards of professional and personal conduct and within the regulations governing the Data Protection Act. . Provide reassurance and receive information. . Ask and instruct relatives / carers for assistance with methods of immobilisation

Radiography staff (internal / external):

. Consult senior staff for advice. . Delegate tasks to Radiographic Helpers and Clinical Support Workers. . Communicate with all staff on any aspect that affects the daily management of the department. . Training of Student Radiographers. Providing constructive criticism as part of the assessment process. . Pass on patient information when transferring patient care to colleagues.

Medical Staff / Other Health Care Professionals: . Advise/ discuss incorrect or unnecessary referrals in order to reduce patient radiation dose. . Liaise with medical and nursing staff e.g. to ensure patient arrive at allocated time, are correctly prepared for examination and using the appropriate mode of transport. . Provide advice on guidelines for relevant x-ray examinations. . Provide advice on the nature of an image. . Seek help and advice when necessary.

10. DEMANDS OF THE JOB (physical, mental, emotional and working conditions)

Physical Skills:

. Positioning of patients demands a high degree of accuracy to minimise radiation dose i.e. avoid repeat exposure and to demonstrate the correct anatomical features required. . Highly developed hand to eye coordination is required when manipulating X-ray equipment and images. . Trauma situations emphasise the need for speed and precision, as well as accuracy in positioning patients. . Positioning patients for examination requires the operator to manipulate the x-ray tube, patient and film, at the same time, whilst constantly assessing the condition and needs of the patient. . For mobile units there is a requirement to precisely manoeuvre large, unwieldly equipment in restrictive conditions, whilst maintaining sterile ‘no touch’ constraints. These units must then be positioned using multi-planar locks. This requires significant eye to hand co-ordination and spatial awareness. . Possess significant keyboard skills for the entry of data into the RIS, CR and PACS. . Transferring and positioning patients for all radiological examinations demands an on-going high level of physical strength and fitness. . Possess basic life support skills.

Physical Demands:

. Ensuring a safe environment for both patients and staff requires that all heavy lead lined doors be closed and locked for each examination. . Maintain a level of physical fitness to frequently move ceiling mounted X-Ray tubes throughout three dimensions during all shifts. . Walk long distances when driving mobile X-Ray units to carry out ward radiography throughout a shift when required. . The majority of the working day and night is spent standing and walking and bending to position patients. . Cleaning of X-ray equipment regularly and changing mattress sheets involves bending and lifting. . Daily transfer of patients from trolleys, beds and chairs onto x-ray tables requires the use of safe lifting and handling skills, with or without the use of mechanical aids when required to ensure patient and self safety. . Carry large cassettes to and from the x-ray facility to the processor or CR Reader. . Required to be on feet all day, often wearing heavy lead coat. Example: in theatre, barium room, mobiles.

To independently meet the high work rate demands of Accident and Emergency whilst also covering CT, portables and urgent inpatient referrals when providing an out of hours service.

Mental Demands:

. Intense concentration is required when assessing patient’s condition and performing examinations to a high diagnostic standard throughout every shift period. . Prioritising workload requires diplomatic skills in discussion with referrers who all believe their patient should take priority. . When providing an out of hours service take responsibility for the radiography service, managing patients, relatives, staff and equipment. . Should imaging equipment malfunction during a shift, evaluate the situation and provide an immediate solution as per protocol. The patient must be informed of the situation. The diagnosis of the patients condition is delayed. This in turn increases both the physical and emotional demands of the Radiographer. . Cope with the mental and physical demands of working in acute areas, independently, sometimes having to provide images on severely injured, abusive or violent patients.

Emotional Demands:

. Perform radiographic examinations and care to terminally ill patients. . Care and Understanding is required when dealing with patients who may be anxious, distressed or terminally ill. . Provide examinations in traumatic circumstances for critically injured patients in the accident and emergency department, sometimes bleeding heavily with distorted or severed limbs and/or severe burns. . Move quickly from a routine examination to an acute setting due to sudden and unpredictable demand e.g. x-ray of a painful finger with no history of trauma to a victim of a road traffic accident requiring multiple X-rays, independently taking responsibility for the diagnostic imaging service to that patient. . The radiographer will deal with a variety of patients from neonates to geriatrics, including mental health patients, prisoners etc, requiring sensitive handling in all situations.

Daily Working Conditions:

. Dealing with unpleasant odours, uncontained body fluids and infections frequently throughout all shifts, therefore requiring the use of universal infection control precautions. . Frequent exposure to verbal abuse from patients/ carers, which can be in a lone working situation with little or no support. . Occasional risk of physical abuse from patients/ carers, which can be in a lone working situation with little or no support. . Be required to wear heavy lead rubber aprons during Fluoroscopy Procedures sometimes in a hot theatre environment. . Risk of exposure to scattered Ionising Radiation particularly during fluoroscopic procedures. . Moving from cold air-conditioned areas to hot air conditions in the working areas. . Working constantly in artificial lighting with little or no natural daylight. . Exposure to verbal abuse from some patients.

Ensuring patient safety for the duration of their time in the x-ray department. Should a situation arise where the patient appears to be a potential danger to themselves or others, the necessary measures would be taken. . Required to carry out mobile examinations outwith the main hospital. This involves going outside alone, often at night. . Periods of time spent in front of VDU’s processing data. . Working with equipment that has constant low-level noise.

11. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB

. Aware of how quickly controlled and stable situations can escalate to become emergency, life threatening conditions. . When working with ionising radiation, accuracy is of primary importance with a narrow margin for error. . Dealing with patients who may become emotional and/or aggressive. . Balancing mental and physical speed and accuracy ensuring that the service to patients is never compromised. . Be prepared to operate Imaging equipment in differing and demanding environments on a daily basis. . Be able to manage an unpredictable workload effectively and interact successfully with fellow healthcare professionals. . Cope with the mental and physical demands of working in acute areas independently, sometimes having to obtain images on severely injured, abusive or violent patients. . As one of a limited number of Radiographers on duty in the hospital overnight meet the high work rate demands of Accident and Emergency whilst also covering CT, portables and urgent In Patient referrals. . Exposure to cases where the patient's prognosis is poor. . Combining training in new techniques or newly procured equipment with normal patient workload. . Keeping up to date with Continuing Professional Development therefore always improving the service provided. . Frequently working with unpleasant conditions such as body odour, bodily fluids, excrement and contact with infectious or contagious materials

SMOKING: Staff are not allowed to smoke in:  The building and grounds of all premises from which NHS Lanarkshire services are delivered, all premises owned or occupied by NHS Lanarkshire and all vehicles parked on these premises  NHS Lanarkshire vehicles and pool cars at all times  Leased cars and personal vehicles when transporting other staff or patients within working hours and/or when on authorised business. Staff may smoke only during unpaid official breaks (normally lunchtimes) and only in line with the paragraph above.
