Role Details Job Title: Advanced Nurse Practitioner Salary Grade: £39,632 to £47,559 per annum (full time) Team: Urgent Care Primary Location: Latham House Medical Practice, Melton Mowbray and upon occasions at the Practice’s Asfordby site. Responsible to: Lead Nurse/Practice Manager Responsible for: Urgent Care Team

Role Purpose 1. To work autonomously as an Advanced Nurse Practitioner providing care to patients requesting an urgent appointment on the day. 2. You will work at an advanced level and provide assessment/diagnosis/treatment referral and discharge of patients presenting with a wide range of undiagnosed/undifferentiated conditions, without the need for Doctor involvement in the majority of patient contacts. 3. Manage, monitor and maintain the efficient and continued development of Urgent Care Services, and needs led services, within the Practice. 4. Manage and support clinical practitioners to ensure the consistent delivery of high quality care within the Urgent Care Clinic.

Role Responsibilities Clinical: • Use high level decision making skills. • Undertake consultation, ensuring patients receive high quality care, delivered in a timely manner. • Maintain accurate and contemporaneous records using the practice clinic system (EMIS web). • Undertake comprehensive history taking and physical examination and request interpret results and investigations in order to reach a diagnosis for patients with undifferentiated/undiagnosed conditions. • Prescribe appropriately in line with local guidance. • To offer health promotion advice and sign post to appropriate agencies when required. • Make direct referrals to secondary care, mental health and social services within locally agreed pathways. • Protect and promote the health and well-being of patients, their relatives and carers, by receiving, triaging. • Work collaboratively with stakeholders across the Health and Social Care sectors, and in partnership with patients and/or carers, develop effective, efficient and preventative care plans.

Professional Role • Maintain an up to date professional registration. • Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review. • Maintain own clinical/professional expertise by participating in clinical activities, personal development planning, clinical governance and revalidation. • Participate in continuing professional development opportunities to ensure that up-to-date evidence- based knowledge and competence in all aspects of the role is maintained. • Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. • Work in accordance with NMC code. • Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation. • Demonstrate leadership within the Urgent Care Clinic. • Pro-actively promote the role of the ANP within the Practice, and externally to key stakeholders and agencies. • Promote a professional approach at all times by dress and attitude to patients and colleagues. • It will be necessary to attend and contribute to various practice meetings as requested. (The only reason for not attending will be annual, study or sick leave). • Develop referral pathways with tertiary services to enable the Urgent Care Clinics to be led by ANP in future.

Communication • To be aware of data protection issues. • To liaise effectively with other health professionals both inside and outside the organisation, with regard to accepting and making clinical referrals. • Communicate effectively with patients and relatives, considering differences in language, literacy and culture.
