Individual A is a Transitioning Student with Intellectual Disabilities, Autism, and Mental/Behavioral Health Diagnoses.
Individual A is receiving services through Special Education, Mental/Behavioral Health, and Children and Youth. Individual A is not yet connected with OVR or Autism Services.
A team has been developed to fully assess and support Individual A. The specific needs of Individual A are at the center of team development, discussion, and service planning.
Team Members: 1. OVR Counselor assigned Individual A’s School District 2. Special Education Teacher from Individual A’s School District 3. Work Experience Coordinator from Intermediate Unit connected to Individual A’s School District 4. Individual A’s Supports Coordinator (ODP – County Level) 5. Individual A’s Children and Youth Case Worker 6. Individual A’s mental health counselor 7. Individual A’s Discovery Consultant/Job Coach 8. Autism Supports Coordinator or other “expert” in Autism to help inform decision-making and service planning, even though this individual is not yet receiving services from the Autism Waiver.
Sample Team #2
Individual B recently graduated high school and was planning to receive services in a sheltered employment setting. Individual B is diagnosed with Intellectual Disabilities, Mental/Behavioral Health Diagnoses, and other medical conditions affecting physical mobility.
Individual B was referred to OVR by his/her ODP Supports Coordinator and is waiting to participate in the Discovery Process and some Community-Based Work Assessments. Individual B also receives services through the Independence Waiver.
A team has been developed to fully assess and support Individual B. The specific needs of Individual B are at the center of team development, discussion, and service planning.
Team Members: 1. Individual B’s assigned OVR Counselor a. If this specific VR Counselor is already on another team, the counselor’s supervisor, an Early Reach Coordinator from the office, or other appropriate OVR staff member may serve as a substitute representing the “mandatory” OVR team member) 2. Special Education Teacher from Individual B’s School District 3. Nurse from Individual B’s School District, previously providing 1:1 supports 4. Individual B’s Service Coordinator from the Independence Waiver 5. Individual B’s Supports Coordinator (ODP – County Level) 6. Support staff from the local Sheltered Employment Facility 7. Individual B’s mental health counselor 8. Individual B’s Occupational Therapist 9. A local assistive technology specialist
