VIRGINIA BOARD OF NURSING INFORMAL CONFERENCES March 31, 2015

TIME AND PLACE: The informal conferences held before an Agency Subordinate of the Board of Nursing were convened at 9:10 A.M., on March 31, 2015 in Board Room 1, Department of Health Professions, 9960 Mayland Drive, Suite 201, Henrico, Virginia.

AGENCY SUBORDINATE: Jane Elliott, R.N., Ph.D.

STAFF PRESENT: Gloria D. Mitchell-Lively, R.N., M.S.N., M.B.A

OTHERS PRESENT: Amy Weiss, Adjudication Specialist

CONFERENCES SCHEDULED: Lee Ann Stone, R.N., 0001-110847 Ms. Stone did not appear.

Based upon information provided by Ms. Weiss, Dr. Elliott ruled that adequate notice was provided to Ms. Stone.

A recommended decision will be made and mailed to Ms. Stone within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order.

CONFERENCES SCHEDULED: Maria Ann Hryhorenko, RN, 0001-244265 Ms. Hryhorenko appeared. Present to observe were Wendy Welch, Nurse Manager Assistant and Mandeline Smith, Human Resource Generalist from Chesapeake Regional Medical Center.

Ms. Hryhorenko was informed that a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Hryhorenko unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Hryhorenko within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.

CONFERENCES SCHEDULED: Holly Ann Kendig, R.N. 0001-230645 Ms. Kendig appeared, accompanied by her attorney Carol Schriefer.

1 Ms. Kendig was informed that a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Kendig unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Kendig within such time. CONFERENCES SCHEDULED: Sigrid Christina Butler, R.N. 0001-190381 Ms. Butler did not appear.

CONFERENCES SCHEDULED: Laura Anna (Bundick) Spence, R.N. 0001-201534 Ms. Spence appeared.

Ms. Spence was informed that her case will be closed due to insufficient evidence to warrant disciplinary action by the Board of Nursing.

ADJOURNMENT: The conferences were concluded at 3:51 P.M.
