1. Questionnaire Administration (Type your response here) Mother Teresa Catholic Academy administered the AdvancED surveys to students and staff in the month of December 2016. The response rates were well above the minimum required: 100% of staff and 98% of students answered the surveys. Since the response rate for students is well above the minimum required for students, there were no make-ups for the 2% of students that were absent on the days the survey was administered. The survey link was sent to instructional staff by email with directions to answer the survey within 48 hours. Students and instructional staff answered the survey online, and none required a paper survey. Since the response rate is well above the minimum required, the survey results represent the overall school.

The standard with the highest level of satisfaction of approval varies for each stakeholder group; the faculty identify ‘resources and support systems,’ the students in grades 3 to 5 identify ‘purpose and direction,’ while students in grades 6 to 8 identify ‘governance and leadership.’
