Attn: All Lessees and Vendors Re: Proof of Insurance Submission Checklist
To help facilitate the submission of your required insurance documentation to meet your contract obligations with the Freeman Coliseum and Expo Hall, we have provided the following checklist for you and your insurance broker.
Please make sure all of the following items are included when submitting your insurance documentation.
Include a certificate of insurance (Accord Form 25)
The certificate must include:
Commercial General Liability Coverage with the required limits of Coverage (follow sample certificate)
Commercial Auto Liability Coverage with the required limits of Coverage (Hired & Non-Owned Auto is ok if there are no “owned” autos)
Worker’s Compensation Coverage with the required limits of coverage
The certificate holder in the bottom left box should read: Coliseum Advisory Board dba Bexar County Community Arenas Board and Bexar County, 3201 E. Houston, San Antonio, Texas 78219
In addition to the certificate of insurance, you must also submit the following Forms (please ask your broker if you are not familiar with these forms)
An endorsement referencing the certificate holder as an additional insured on the general liability policy and the auto coverage. Either on a blanket basis as required by written contract or as a specific endorsement listing the certificate holder as listed on the certificate.
A waiver of subrogation endorsement in favor of the certificate holder on the general liability policy and the auto coverage. Either on a blanket basis as required by written contract or as a specific endorsement listing the certificate holder as listed on the certificate
A waiver of subrogation endorsement in favor of the certificate holder on the workers compensation policy. Either on a blanket basis as required by written contract or as a specific endorsement listing the certificate holder as listed on the certificate
***Please submit your certificate only if you have all of the additional forms attached. A certificate of insurance without the required forms attached is insufficient to meet your contract obligations and could prevent access to the premises on your scheduled occupancy dates***
