Note: In determining actual brackets, teams will be placed on a first come - first served basis. Once a bracket fills out, teams joining that bracket will be wait-listed or given the opportunity to go in another bracket. After the deadline, brackets will be opened and adjusted if any spots are available. Wait listed teams will be placed accordingly. If a bracket is not filled, we will combine another bracket within the division (teams will be notified). As a reminder, an individual player MAY ONLY PLAY ON ONE TEAM, regardless of Division.

Please provide 2 volunteers per team entry to assist as Field Marshalls for a few hours each during LOCAL TEAMS the tournament Name Phone Email Name Phone Email NEPTUNE SOCCER CLASSIC is an affiliated tournament that is sanctioned by the United States Soccer Federation (USSF) and the United States Adult Soccer Association (USASA) via the local state association, the Metropolitan DC-Virginia Soccer Association (MDCVSA). All tournament players must be registered in a league which is affiliated with the USSF, or a foreign federation recognized by USSF. ▪ Tournament teams must fit into one of the following categories ▪ IN-STATE Your team consists of affiliated players from MDCVSA member leagues and teams. You may register up AFFILIATED to seven new players who were not formerly registered with MDCVSA or another US Soccer entity. TEAMS Registration of new players must be done directly through MDCVSA, and will cost $25.00 (USD) per player. Guest Players will register online (instructions will be provided). All MDCVSA teams must submit a preliminary roster to MDCVSA by July 19th via email ([email protected]) In your cover email, include your name, phone number, state association, home league, team name, and tournament division. Email the roster to [email protected]. Within three business days of your submission, you will receive a reply email confirming those players who are registered with the MDCVSA. Any discrepancies (such as players missing on the MDCVSA database) will be noted. The version of the roster sent back to you from the MDCVSA is the version you must use to make any changes going forward. If you have unexpected roster changes after July 19th, you may submit ONE revised roster to [email protected] which must be received by noon August 1, 2017. After August 1st, up to 4 players may be added to your roster at registration. However, teams with little or no roster changes will be sent through the express line, so it is in your best interest to have your roster up-to-date and verified prior to registration. Players will be verified using a valid I.D. (driver’s license, passport). Proof of I.D. will be verified prior to every match!

OUT-OF-STATE Teams already affiliated with an existing USASA state association (or other US Soccer entity) are AFFILIATED permitted to play in the tournament. Your state association must provide the Neptune Soccer Classic with TEAMS an approved roster that includes the names and birth dates of your players (note: this correspondence must come directly from your state association). All players should be registered through your state association and included on the official roster. (International teams are also permitted, as long as you submit a Travel Permit and roster from your national soccer association.) Out-of-state teams should have their state registrar submit an approved roster that includes the names and birth dates of your players to the NSC no later than July 19, 2017. Your state registrar may submit approved rosters to [email protected]. Plan to bring a copy of your approved roster submitted by your state association to registration.

OUT-OF-STATE Unaffiliated teams that come from geographic areas outside the MDCVSA must contact their home State UNAFFILIATED Association to inquire about affiliation status. You may obtain contact information for your state association TEAMS by visiting www.usasa.com or www.ussoccer.com . Contact the NSC if you have additional questions at [email protected].

One of the benefits of being an affiliated player is the secondary player medical insurance offered through the USASA. To receive the benefits of the secondary player medical insurance, you must properly complete an insurance claim form, sign it, get it signed by your league representative, then submit the completed, original, signed form to the MDCVSA Administrator. All medical insurance claims must be filed through the MDCVSA. Your team captain, coach, or league representative cannot approve an insurance claim, although those persons may be required to sign the claim or provide information in support of the claim. Any injured player should be sure to obtain a copy from the tournament Trainer, signed by a NSC Tournament Committee Member as confirmation the injury was sustained during the NSC. Include this with the claim submission to the MDCVSA Administrator. Team Captains – please refer any player who gets injured during a match and wishes to file an insurance claim to the following link on the MDCVSA web site: http://www.mdcvsa.org/insurance.html
