 Child / Young Person CAF/Self Evaluation Pages 5-6 This is a tool for the Child / Young person to engage in the process and puts across their views and perspectives. This can be used as an impact assessment to see if changes have taken place for the child/young person throughout the interventions. This will also support completing the Distance Travelled Tool with the parents as it will provide the child’s perspective.

 Child / Young person TAF Plan- My TAF Page 7 This is the child / young person’s actions written in a child friendly way for their own beneficial record. Expressing the views, aims and outcomes of the meeting.

 Team Around The Family (TAF) Minutes Pages 8-11 This is the paper work for the Team Around the Family (TAF) Meeting. It supports and identifies the action plan for the child / young person and their family. The responsibility of taking the meeting minutes should be given to a member of the TAF meeting and should not be the Chair (Lead Professional).

 TAF Closure Notification Page 12 This form needs to be sent back to the Gateway following the closure of the TAF meeting

 Distance Travelled Tool and Flash Cards Page 13 This is the tool that is used alongside the TAF meetings in order to evidence impact of multi-agency working in meeting the additional needs of children and families. The questionnaire works in conjunction with the flashcards attached to the LP Pack.

 TAF Questionnaires Pages 14-15 These questionnaires are an opportunity for a child or young person to share their views of the TAF process. Please complete these forms and return them to [email protected]

Distance Travelled Tool

The journey of the child and whether or not our services support the family is critical. This is now monitored corporately by using the Distance Travelled Tool to assess impact. As each new case is identified and the Family CAF is sent out the expectation is that the lead professional will carry out the Distance Travelled Tool as part of the role.

In your capacity as a lead professional (LP) you are required to complete the Distance Travelled Tool. This takes place when you first start working with your families in TAF that you are LP for. All you have to do is sit down with the family at the start of the intervention and complete the questionnaire based on the needs of the family as a whole. If there is more than one child who the family are struggling with and require additional support you might want to carry out separate Distant Travelled Tools. Give the young person or parents, carers or guardian the flashcards (prompt cards) and ask the 20 questions. Place the score in the box which best describes the negative effect each factor is having on them. Then add up the total of each column and enter the results on the bottom of the page. Over time as the issues lessen due to the agencies supporting them the figure should reduce.
