Students that are applying early action/early decision must submit this packet by October 1, 2013. Students that are applying regular decision must submit this packet by November 1, 2013. Dear Parents and Students,

Seniors who will be applying to private universities may be asked to provide letters of recommendation as a part of the college application requirement. Universities of California/UCs or California State Universities/CSUs do not require a letter of recommendation as a part of their application process. In order to help facilitate the large number of requests that the counselors receive, we have designed a packet to assist us in the process. Requests are processed in the order they are received. Processing time is dependent on the number of student requests.

A large number of private universities use the Common Application or SENDedu. As part of the application, counselors complete a secondary school report (includes student’s GPA, electronic copy of transcript, school information, etc.) as well as a written evaluation of the student. Counselors prefer students to apply through the Common Application for colleges that use both SENDedu and the Common Application.

Students that are requesting an evaluation from their counselor need to:

 1. Request your counselor online (on the Common App it is under the forms tab). *make sure you spell your counselor’s name correctly and have their e-mail address right* (your counselor cannot accept your request packet until this step is complete. See below for contact info.)  2. Submit to your counselor a hard copy (no e-mails) of your request packet by October 1, 2013 if you are applying early action/early decision or November 1, 2013 if you are applying regular decision.

Packet must include: o This completed checklist o Student Information Form AND a list of activities/awards o A copy of your personal statement (optional) o Unofficial copy of all SAT, ACT, or AP test scores from collegeboard.com or ACT.org (These will not be given to the colleges you are applying to. OFFICIAL SAT or ACT scores need to be submitted to the college by the student if the college is requesting them).

*For students that are applying to private schools that do not use the Common Application: Submit this packet and whatever documents are needed by your college to your counselor. You will need to pick up the documents after they are completed by your counselor and mail them to the appropriate person or office. If an official transcript is required by your college, you need to request it from Ms. Lankford in the main office.
