Application Form for the Supplier Diversity Excellence Award

The Vizient Supplier Diversity Excellence Award recognizes the member that best champion’s inclusion in their supply chain and accelerates economic growth in their local community through the engagement and empowerment of minority-, women- and veteran-owned business enterprises.

Criteria • Growth of member’s supplier diversity program in 2017 • Strength of community involvement and outreach • Senior leadership support of member’s supplier diversity program • History, focus and uniqueness of member’s supplier diversity program • Use of Vizient diversity contracts

Application Questions and Weighting Responses to the questions below should not exceed 8 pages total. We suggest one and 1/2 page per question response.

1. Does your organization have a formal supplier diversity program with established utilization goals? If yes, please describe the program’s history, focus and provide results of measurable progress (10%).

2. What sets your organization apart? Specifically, what new techniques or programs did your institution implement in 2017 to facilitate growth and development of minority-, women-, and veteran-owned business enterprises? (25%)

3. Describe your organization’s utilization of Vizient’s diversity contracts (25%)

4. Describe your senior leader’s involvement in supplier diversity development (15%)

5. Describe your organization’s community involvement and outreach to minority, women, and veteran owned business enterprises. Please provide a roster of all local (within 250 miles) diverse suppliers, addresses, products, services, and purchase amounts of these suppliers Jan. 1 - Dec. 31, 2017. (25%)

Supporting Materials You may include additional documentation and collateral to support your award submission, including but not limited to strategic planning and/or goal-setting documents, new releases, marketing and outreach collateral, program best practices, documentation of awards received, spend reports, etc. Please include no more than 15 pages of supporting material.

Nomination Process Eligible Vizient members will apply for consideration during the 2018 Vizient Excellence Award call for entries (Oct. 23, 2017 – Feb. 15, 2018).

Deadline Entries must be received by midnight, Feb. 15, 2018. Winners will be notified by March 15, 2018. Please submit your entry electronically to [email protected].
