Dear Patient

Welcome to St Chads and Chilcompton Surgery Online Services.

When you register for this service you will be asked to complete an application form and provide photographic identification (e.g. Driving Licence, Passport etc.) for verification purposes. The whole process will be explained to you and you will then be given a user name and password. You will need to visit the surgery to do this but the process should only take about 10 minutes. Your details will be personal to you and cannot be used for anybody else.

Please be aware that you can only order medication that is on your repeat slip. You will need to allow 48 hours if you will be collecting the prescription from the surgery and 72 hours to collect medication from any chemist to which you request your prescription to be sent.

You can book an appointment online but should you be unable to attend that appointment please remember to cancel it (this can also be done online) as appointments are always in demand and it can be made available for others. The practice has a policy regarding patients who fail to attend appointments they have made and anyone failing to attend on 3 occasions can be removed from the practice list.

It is also now possible to access Online Services via a smart phone app. Search for SystmOnline in the app store on i-tunes or Google Play for the Android version to download the free app.

Any online user that reaches the age of 11 and/or 16 years will have to re-register for this service on or after their birthday.

If you have any queries or problems when you are using the system, please contact reception.

Before you apply for online access to your record, there are some other things to consider.

Although the chances of any of these things happening are very small, you will be asked that you have read and understood the following before you are given login details.

Things to consider:

 Forgotten history – there may be something you have forgotten about in your record that you might find upsetting.

This may occur before you have spoken to your doctor or while the Surgery is closed and you cannot contact them, in some cases you may see your results before the GP.

 Choosing to share your information with someone – It is up to you whether or not you share your information with others, perhaps family members or carers. It is your choice, but also your responsibility to keep the information safe and secure.

 Coercion – if you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.

 Misunderstood information – your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record may be highly technical, written by specialists and not easily understood. If you require further clarification, please contact the Surgery for a clearer explanation.

 Information about someone else – if you spot something in the record that is not about you or notice any other errors, please log out of the system immediately and contact the Practice as soon as possible.
