Application Form External Party Access to DHA Online Services

Introduction The external party to Defence Housing Australia (DHA) must read and accept the attached terms and conditions before being granted access to DHA Online Services.

What to do next? 1. The external party is to complete and sign the attached application form. 2. The completed form is to be emailed to the appropriate business contact (Client Services, Operations, Sales) at DHA.

3. The USERID and password will be sent automatically to the external party email address once the access to Online Services has been established.

External Party access to DHA systems. 1. DHA agrees to provide the External Party limited access to Online Services subject to the individual External Party agreement to and compliance with the following conditions. 2. By signing these conditions, the External Party acknowledges that DHA may, without notice, cancel this access if DHA becomes aware of any non- compliance with these conditions by the External Party, or as a result of any unauthorised use of the USERID. In such cases DHA is under no obligation to restore Online Services access for the user.

3. Background information: a. Property, Tenancy and related information on DHA Online Services is the confidential information of DHA. DHA provides External Party’s with access to information in accordance with DHA protocols. Access to all such information is at the discretion of DHA. b. Access to Online Services increases the information available to the External Party, and requires External Party strict compliance with these conditions. c. Access to Online Services for any business purpose is strictly controlled according to their individual circumstances, and the data provided varies accordingly. Interpretation of data provided online must be made by DHA staff trained in appropriate business functions.

4. The Conditions of Access to be agreed are as follows:- a. This access is provided on the condition that it will be used by the External Party’s only for their own familiarisation of DHA housing. b. Users granted External Party access must notify DHA when they cease to hold the responsibilities of an External Party. c. External Parties must not allow other people, including DoD personnel, to view DHA Online Services via their USERID. d. External Parties must not pass on any housing or other information on DHA Online Services to other personnel by any means. This information remains confidential to DHA. e. External Parties agree to familiarise themselves with the other Terms and Conditions as provided on DHA Online Services and agree to comply with these conditions. f. Should a situation arise where the External Party believes that a departure from these terms is justified, he/she agrees to obtain DHA’s prior written approval from DHA, before undertaking any departure.
