Parent/guardian signature on registration form grants permission to participate in the Hoops by the Lake 3 on 3 Basketball Tournament. As the Parent/Guardian, I am aware of and accept the risks involved in this experience and hereby release and discharge the Hoops by the Lakes 3 on 3 Tournament, tournament officials, tournament directors and other participants, from all claims, demands, actions, judgments, and executions which the signed names on the registration form ever had, or now has, or may have, or claim to have. This includes all personal injuries, known or unknown and injuries to property, real or personal, caused by, or arising out of the above-mentioned basketball tournament.

I, the person(s) signing the Registration Form have read this release and understand all its terms. I execute it voluntarily and with full knowledge of its significance.

For players over 17 years of age, there will be a release form to be signed upon showing up at the tournament.
