1. Fill out your information: name (first and last), phone number (home and cell), address (preferably in Chinese if you can), email, and pet’s name(s) * It is very important to indicate any allergy, if any 2. Indicate the amount and kind of biscuit you want and calculate the cost of each (dog and cats) 3. Choose your payment type that you prefer a. Pre-pay: you pay when you turn in the order form b. Pay upon delivery: you pay when the biscuits are delivered to you c. Pay in advance: you can deposit X amount of money and we’ll deduct the money from your advanced payment each time you order 4. Choose the type of delivery a. Through your child / yourself at SAS Pudong *Indicate your child’s / your homeroom and grade b.
