Conference Site Connection and Testing: http://webcast.bcm.edu/txch/transitions17/player.html There will be two broadcast options, YouTube or Live Stream. Both options will display the audio, video, and slides concurrently. There will not be any telephone feeds. Choose the most optimal streaming option for your site by testing each using the instructions below.

Testing Instructions 1. YouTube a. Click on the site connection link above b. Click on “Watch live via YouTube” at the bottom of the page. If the test video plays, this option works for you. 2. Live Stream: The live stream will only play during the conference. a. Mark your calendar to perform a live stream test on Wednesday, October 12, 2016 or Thursday, October 13, 2016, between 9:00 am – 4:00 pm (CST). b. Click on the site connection link above c. Click the “Play” icon. If the live stream works during the testing window, you will be able to receive the conference broadcast signal at the time of the conference.

Accreditation/Credit Designation: Physician Baylor College of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. Baylor College of Medicine designates this live activity for a maximum of 14.50 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. The presentation, Legal Issues in Health Care Transition, has been designated by Baylor College of Medicine for .75 credit of education in medical ethics and/or professional responsibility. Social Work Continuing Education Units will be available for social workers. Occupational Therapy and Physical Therapy Contact hours (CCUs) have been applied for through TPTA and TOTA. Texas Children’s Hospital is an Approved Provider for the Texas Occupational Therapy Association. The assignment of TOTA CE hours does not imply endorsement of specific course, content, products, or clinical procedures by TOTA.
