NHS FIFE OPERATIONAL DIVISION

CONSULTANT IN UROLOGICAL SURGERY REF NO:

Applications are invited for two Consultant Urological Surgeon posts to work with a team of 5 Consultants at Fife NHS trust. These will be new posts within the service and it is expected that the post-holders will have a thorough training in all aspects of General Urology and be expected to complement existing sub-specialty interests of the team. Subspecialist skills in Upper Tract Laparoscopic Surgery desirable for at least one of the posts.

The urology team within NHS Fife is dynamic and has a commitment to building excellent care for our patients through unique developments of our service. The team consists of 5 Consultants, 1 Specialty Trainee, 1 Specialty Doctor, 4 Clinical Nurse Specialists and a dedicated nursing team for the Urology Diagnostic Centre (DTC).

NHS Fife delivers a high quality service to a population of approximately 380,000. The Urology service is delivered on two main hospital sites. In patient urology wards and theatres are located in a new facility that opened in early 2012 in Victoria Hospital, Kirkcaldy. The department provides a comprehensive urology service with open and laparoscopic renal surgery, open pelvic surgery, reconstructive and incontinence surgery, and stone surgery (FURS & laser lithotripsy).

Outpatient services are delivered at Queen Margaret Hospital, Dunfermline. Following significant investment from the Trust, the new Urology DTC opened in October 2014. Our innovative one-stop urology clinic is modeled on the Guy’s Hospital one-stop service and allows for all diagnostic tests to be performed in one visit. In addition, the Urology DTC has a single management structure and is supported by an experienced specialist nurse team – providing enhanced services to our patients such as nurse led endoscopy, ultrasonography and urodynamic investigations.

Additional development at the Queen Margaret site saw the recent opening of the Regional Endoscopy Unit and the new Day Case Surgery Unit. The Urology department has recently completed a 5 month trial of Greenlight Laser Prostatectomy. The department is planning to offer this new service at the Queen Margaret site and utilising the increase capacity of 23hr stay beds at the new Day surgery Unit. The department is actively involved in undergraduate and postgraduate education and training and affiliated to the Universities of Edinburgh, Dundee and St. Andrews. The department hosts students from all three universities throughout the year. We have developed strong research links with the School of Medicine at the University of St Andrews. Research activities in the department are encouraged and are well supported by the R&D department who has new resources such as the new Clinical Research Facility in Victoria Hospital.

It is anticipated that the emergency Urology Consultant care will be shared on a 1:5.8 contract.

You will be on the Specialist Register of the GMC or be eligible for inclusion on the Register within six months of the time of interview.

Fife is the ancestral home of the Scottish monarchs, with a wealth of history found in its castles and cathedrals. Fife is world famous for its golf, and there are abundant leisure facilities with fishing, sailing, hill walking and other rural pursuits set in beautiful surroundings. Victoria Hospital is a large fully equipped District General Hospital.

First opened in 1966 the hospital has been further expanded and modernised to become in January 2012 the Acute Inpatient Site for Fife.

 The theatre complex in Phase 3 consists of 11 theatre suites.  Adjacent are the Intensive Care Unit, Medical HDU, and Surgical HDU.  The Day Intervention Unit, housing Endoscopy, Interventional Radiology and Day Surgery, is also adjacent to the theatre complex in Phase 3.  In Phase 2 there are 2 dedicated Orthopaedic theatres.

Radiology

There is a comprehensive range of inpatient and outpatient services. The radiology department is spread over 3 units:

Diagnostic Imaging Suite:

It consists of CT scanner, an ultrasound room and 2 DR x-ray rooms.

Interventional Suite:

Situated in the Day interventional unit, which is part of the operating theatre complex.

Main Department:

A mixture of in-patients and out-patients are seen in this department. It consists of CT scanner, MRI scanner, 2 fluoroscopy rooms, 2 ultrasound rooms, nuclear medicine, DEXA and a dental room.

Radiotherapy/Oncology

Oncology services are provided from Western General Hospital, Edinburgh. Clinical oncologists with special interests in GI, urology, lung and gynaecological cancer hold large outpatient clinics each week.

Library

There is a well equipped library with CD ROM databases (Medline 1966-data, Cinahl, Best Evidence, National Research Register, HEBS on CD). There is access to the Internet and Word Processing (Word 97, PowerPoint version 97 and Excel version 97). Training in the use of CD-ROM and Internet is available from the Librarian. Interlibrary loans and photocopying is available.

Regional Laboratory Service

The Regional Laboratory is on site. The department provides a comprehensive service in haematology, immunology, histopathology, cytology, microbiology and clinical chemistry on a Fife-wide basis. The Microbiology Department provides an overall microbiological surveillance system throughout the region. Queen Margaret Hospital

Queen Margaret Hospital became a Diagnostic and Treatment Centre as of January 2012 as part of the reconfiguration of health services in NHS Fife. Over the coming months Fife will be enhancing day case surgical provision on this site as well as refurbishing endoscopy suites. There will be increasing redesign and transformation of the general outpatient services offered to patients in Fife from this site.

A new Urological Diagnostic and Treatment Centre opened in October 2014. This centralised and integrated all Urology specialist clinics, OP clinics, TRUS lists, flexible cystoscopy lists and day case theatre. It has a single management structure and is supported by a specialist nurse team and secretarial staff.

There are 4 main theatre suites providing day surgery across a range of specialties as well as a fully equipped endoscopy unit which is just about to move to a new fully refurbished accommodation. (c) Radiology

The department consists of:  Three general rooms.  One fluoroscopy room.  One CT scanner  One MRI scanner  One mammography suite.  One dental unit.  Three ultrasound rooms.

(d) Urology Centre

This newly commissioned Diagnostic and Treatment Centre for Urology is provisioned for all clinical diagnostic and minor urological intervention procedures. The core activity being the One Stop Clinics with facilities for flexible cystoscopy, prostatic biopsies, scrotal ultrasounds and uroflowmetery. The Centre also hosts Review Clinics, Nurse-led clinics (including Erectile Dysfunction Clinic), bladder instillations, trial without catheter service etc.

(e) Radiotherapy/Oncology

There are close links with the medical and clinical oncologists at the Western General Hospital, Edinburgh. They run regular outpatient clinics on site and visit inpatients.

(f) Library

This local hospital provides GP beds and outpatient facilities for North East Fife. Clinics provided are:

Adamson Hospital, Cupar

This small friendly local hospital provides a range of surgical outpatient and day care services. The St Andrews Community Hospital and Adamson Hospital, Cupar have an outpatient dermatology service provided by Tayside University Hospitals.

Other hospitals

Other hospitals include Stratheden Psychiatric Hospital near Cupar and a Psychiatric Day Hospital next to the Victoria Hospital. Cameron Hospital at Windygates houses beds for care of the elderly and a young chronic sick unit. There are also GP beds at Glenrothes Hospital. There are three wards at Lynebank Hospital – two long term care wards and one interim care ward for elderly patients. JOB DESCRIPTION

The post-holder will have a thorough training in all aspects of General Urology, be expected to complement existing sub-specialty interests of the team and in time develop a subspecialist interest of their own.

A team of highly trained inpatient and outpatient Nurses DUTIES AND RESPONSIBILITIES

The post holder will join a team of 5 Consultant Urological Surgeons in the Operational Division of NHS Fife, to provide elective and emergency services in Fife. The appointee will undertake commitments at Victoria Hospital, Kirkcaldy and Queen Margaret Hospital Dunfermline.

We are looking for Consultants to complement the interest of the current consultants. You will be expected to fulfill an accepted clinical role as a consultant in the specialty and play an active part in the day to day management of the service as well as that of the surgical directorate.

It is anticipated that the emergency Urology Consultant care will be shared on a 1:6.8 contract.

It is also anticipated that the appointees will take an active role in both undergraduate and postgraduate teaching and training as well as audit and research. Basic Science research links are already established with St Andrew’s University.

You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with consultant colleagues in the medical contribution to management.

Subject to the provisions of The Terms and Conditions of Service you are expected to observe the Division’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters and follow the standing orders and financial instructions of the Division.

In particular where you formally manage employees of the Division you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary.

The attached job plan describes an 8:2 split between direct clinical care and supporting professional activities. NHS Fife allocates all consultants two SPA for CPD, audit, clinical governance, appraisal, revalidation and routine service management meetings. As a centre promoting teaching and research, NHS Fife will often allocate additional SPA time for undergraduate teaching, management, clinical supervision, educational supervision and research. These are areas NHS Fife has a strong commitment and is keen to support. The precise allocation of SPA time and associated objectives will be negotiated after appointment. Review of Job Plan The Job Plan is provisional and still to be finalised and will be subject to review once a year by you, your colleagues in Urology and the Clinical Director for the Surgical Directorate, as laid out in the terms and conditions of the new consultant contract. Out of Hours Responsibilities

The appointee will provide emergency based at Victoria Hospital, Kirkcaldy, on a 1:5.8 on call rota.

Cover for Consultant Colleagues Annual/Study Leave - You will be required to provide emergency cover for consultant colleagues during absence on annual or study leave. If for any reason such deputising is not practicable, Operational Division of NHS Fife undertakes to authorise immediate consultant locum cover.

Supervision of Junior Medical Staff

The post entails that you provide professional supervision and management of junior staff within the department.

Medical Education

You are required to participate in continuing medical education (CME). You are entitled to 30 days paid study leave within any 3-year period, with expenses for the purposes of CME.

Audit

Collaboration with your consultant colleagues, you will be expected to partake in regular audit within the department. You will be provided with the appropriate computer hardware and software along with the maintenance of equipment and secretarial support for such an audit.

Clinical Research

Clinical Research and Unit Meetings - whenever possible, provided it does not interfere with your clinical responsibilities, you should endeavor to attend clinical meetings of the department. Teaching

The post holder will be expected to take part in formal teaching and training od junior staff. In addition, regular nurse/staff educations sessions with consultant in-put are encouraged. Undergraduates from Dundee and Edinburgh universities are regularly attached to the medical firms.

Managerial/Administrative Responsibilities

You are encouraged to attend all meetings of the Urology Department. You may also be required to attend the committees at the behest of the Operational Division of NHS Fife or Royal Colleges or National Committees. The Operational Division of NHS Fife accepts that your commitment to undertake such duties may involve cancellation or alteration of your fixed commitments, as well as expenses.

FIFE REGION

Public Health

The primary task of the Department of Public Health is the analysis of problems of health and health care in Fife and the planning of measures to promote health prevent ill-health and provide health care. The aim of the department is the maintenance and improvement of the health of the community. Public Health Specialists advise the Director of Public Health on specified clinical areas including Acute Services, Mental Health, Care of the Elderly, Physical Handicap, Child Health, Well-Woman & Family Planning, Communicable Disease Control, and Environmental Health.

Health Education

The Department of Health Education supports all health educators in the community, including medical staff, and runs the Board’s Resource Centre and Library.

CHP’s

The Community Health Partnerships are responsible for administration of all Primary Care Services.

This is a devolved service to three Community Health Partnerships (CHPs) delivering Primary Care Services. Community Health Partnerships will seek to bridge the divide between primary and secondary care and between health and social care. By devolving resources and decision making to frontline staff, this enables health and social care professionals to look at the whole picture of care from a patient’s point of view. Postgraduate Medical Education

Dr Morwenna Wood is responsible for the delivery of Regional PG Services and reports jointly to the Medical Director (Dr Gordon Birnie, Associate Medical Director, Fife Acute Services Division) and to the South East Regional Postgraduate Dean (Professor Bill Reid). Both Education Centres (Kirkcaldy at Victoria Hospital and Dunfermline at Queen Margaret Hospital) offer study, IT and library facilities; PG courses, lectures and clinical meetings, as well as ready access to library and PG activities in Edinburgh.

Many units maintain links with the University of Edinburgh and are involved in foundation year and core surgical training schemes, the VTS (Vocational Training Scheme) for General Practitioners and Rotational Training Scheme for Specialist Registrars (South East of Scotland and East of Scotland).

Undergraduate Medical Education

Fife has an ongoing Undergraduate Medical Education Programme in partnership with the Universities of Edinburgh, Dundee and St Andrews. Approx 600 medical students attend for clinical attachment, in various specialties, throughout the academic year. Urology has a major teaching commitment for fourth year medical students with four medical students being attached to the unit at any time

A comprehensive weekly program for introduction to Urology for third year medical students of the University of St Andrews is also well established. Approximately 140 students attend throughout the year HOSPITALS

Fife Hospitals serve a population of approximately 360,000. District General Hospital Services are provided at Kirkcaldy (Victoria Hospital) whilst Queen Margaret Hospital (Dunfermline) now operates as a Diagnostic and Treatment Centre. The nearest teaching hospitals providing regional services are in Edinburgh (Western General Hospital and Royal Infirmary) and Dundee (Ninewells Hospital).
