1. All Managers must register your team via: https://kdsansfc.calfordgreen.com/RegisterOnline/ and submit this Team Nomination form to [email protected] 2. All players, managers , coaches and volunteers must register through ‘My Football Club’ by following this link http://www.myfootballclub.com.au/index.php?id=3 a. Fill in all details and include a photo. b. Complete the Registration page follow the following steps: i. NSFA Summer Football ii. Put what classification you are registering under e.g. player, Match Official, Volunteer iii. NSFA Summer 6aside iv. Select package v. Next c. Follow the questions and fill in ALL mandatory fields. d. Players must fill in the necessary additional questions. e. You will now be registered. 3. The Manager will need to provide this form to [email protected] with the complete list of all players to be allocated to the above team. See page 1 of this form attached. 4. All Team Payments must be made through the Calford Green website or by sending through the payment on the form below no later than 1st September 2014. 5. All Team payments are : a. St Ives Senior (U18 & older) = $1,070.00 including GST b. St Ives Junior = $880.00 including GST c. Mills Park Senior (U18 & older) = $990.00 including GST d. Mill Park Junior = $825.00 including GST Refundable deposit of $100.00 to be paid upon team registration. This is a team fee which includes, “T”shirts for each player, ball for the team, insurance – no additional costs. Colours include: Emerald, Royal, White, Purple, Orange, Red Sky Maroon, Yellow and Black. The choice of colour will be first in for each Age Group division. See order form attached.

Team Payments

e. Credit Card (online payment can be made) when you register or go back in to pay by 1st September 2014. This payment gateway will attract a surcharge of approximately 2%. f. Credit Card payment on this form by 1st September. This payment option will also attract a surcharge of 2%. g. Direct Credit: BSB: 082 201 A/C No: 509465329 (Reference: “your team name”) h. Cash payments will only be taken on the night of the Manager’s Information night on Wed 3rd September (7.00 – 8.30pm) at Asquith Bowling Club (Lodge Street Hornsby). i. All Payments must be received before the start of the 2014 NSFA Summer 6aside season on 9th September except for late entries. Payment for late entries is due when the team is registered.
