The Dutch Kidney Foundation (DKF) General Funding Conditions (‘'Subsidievoorwaarden Nierstichting Nederland'') apply to all awarded DKF subsidies as well as all proposals for DKF subsidies. Applicability of general and other conditions of the applicant, the applicant's institute and of third parties is explicitly excluded.

The execution of a DKF Call for proposals, selection of pre-proposals for full application and positive DKF grant award decisions are conditional on DKF revenues and budgetary means. The DKF explicitly reserves the right to cancel an initiated Call for proposals, to suspend a running procedure or to lower a grant amount in relation to earlier statements.

7 V. PATIËNTENPERSPECTIEF Dit onderdeel van het aanvraagformulier is bedoeld voor de beoordeling vanuit het patiëntenperspectief door een patiëntpanel van de Nierpatiënten Vereniging Nederland. Een vertegenwoordiger van het patiëntpanel zal aanwezig zijn bij de vergadering van de Wetenschappelijke Raad om het patiëntoordeel toe te lichten. Deze paragraaf moet het patiëntpanel helder inzicht geven in de aanvraag. Het doelniveau van de teksten is de algemene bevolking. Deze paragraaf graag invullen in het Nederlands. NB. De term patiënten in deze paragraaf kan ook betrekking hebben op donoren.

Samenvatting ten behoeve van beoordeling vanuit het patiëntenperspectief (max. 500 woorden) Geef een heldere samenvatting van het voorgestelde project met een introductie van het onderwerp en de probleemstelling, eventueel vooronderzoek, hypothese van het onderzoek, methoden en vraagstelling. Omschrijf duidelijk de doelstelling van het project. Vul aan met belangrijke context waar nodig.

2. Relevantie (het belang) van het onderzoek voor de (toekomstige) patiënt (max. 300 woorden) Beschrijf in het kort op welke manier het onderzoek bijdraagt aan een belangrijke behoefte van de patiënt (bijv. verbetering van gezondheid, kwaliteit van leven, maatschappelijke participatie, zelfredzaamheid en zorg).

3. Plan van aanpak (max. 800 woorden) Beschrijf in eenvoudige woorden het plan van aanpak. Geef aan of het onderzoek uitkomstmaten op patiëntniveau gebruikt om gezondheidswinst te meten (bijv. gezondheid, kwaliteit van leven, autonomie van de patiënt/cliënt).

Haalbaarheid plan van aanpak vanuit het patiëntenperspectief (max. 300 woorden) Geef een omschrijving van de belasting van deelname aan het onderzoek voor de patiënt (bijv. het ondergaan van de behandeling/diagnostiek, invullen van vragenlijsten en duur van het onderzoek. Indien van toepassing, kunnen patiënten bezwaar hebben tegen deelname aan het onderzoek (bijv. vanwege een voorkeur voor een bepaalde behandeling)?

5. Informatie en begeleiding voor de patiënt (max. 300 woorden) Beschrijf in eenvoudige woorden op welke manier patiënten informatie ontvangen over deelname aan het onderzoek en op welke wijze de begeleiding van patiënten tijdens het onderzoek geregeld is.

6. Betrokkenheid van ervaringsdeskundigen bij het onderzoek (max. 300 woorden) Benoem de betrokkenheid van ervaringsdeskundigen (of een patiëntenorganisatie) bij het onderzoeksvoorstel. Geef aan of de betrokkenheid terug te zien is in de verschillende fasen van het onderzoek (bijv. opzet van het projectplan, uitvoering van het onderzoek) en wat de faciliteiten hiervoor zijn.
