STEP BY STEP INSTRUCTIONS ON HOW TO APPLY FOR AN NPI NUMBER FOR NON-PHYSICIAN SURGICAL ASSISTANTS (LSA, RSA, KYCSA, CFA, CSA, SA-C) Luis F. Aragon, LSA, RSA

The following instructions are not meant for non-physician surgical assistants such as RNFA, CRNFA, NP, Midwives or Physician Assistants, these providers should utilize their appropriate taxonomy codes.

First you have to log in to the following internet address: https://nppes.cms.hhs.gov/NPPES/StaticForward.do?forward=static.npistart

Having a NPI number:

a) Does not guarantee reimbursement

b) Does not mean that you will be reimbursed by Medicare, Medicaid or other Federal regulated insurance policies .

c) Does not mean that you are an in-network provider unless you contract directly with the insurance company in question.

STEP 1

a) Click on “Apply Online for an NPI” STEP 2:

a) Please read all the information required for individual providers or organization before you begin.

Step 3:

a) Select a user ID and password of your choice, to be able to login again in the future, in case you need to make changes. STEP 4:

a) Fill out all the demographics, especially the fields that have a red asterisk (*) next to it.

STEP 5:

a) Continue to fill out your business mailing address if different from above. STEP 6

a) Continue with business practice location, usually for a surgical assistant it would be the same as your mailing address, especially if you serve several facilities.

STEP 7:

a) For non-physician SAs, this would not apply, just click next to continue to the following screen.

STEP 8:

a) In here is where you have to add your taxonomy code, just click where it says “add Taxonomy”. STEP 9:

a) Choose Provider type code No. 24, which is “Technologists, Technicians & Other Technical Service Providers” and click Next.

STEP 10:

a) In this screen, scroll down to where it says the following: 246ZC0007X – Specialist/Technologist, Other – Certified First Assistant

b) If you are a LSA in Texas, RSA in Illinois or KYCSA in Kentucky, then fill out your license information and State where it was issued. STEP 11:

a) Fill out the contact person form, usually the same as the provider contact information. If you have an office manager, then fill out his/her information.

STEP 12:

a) Check mark the certification about the application.

b) And click “Submit”

STEP 13:

a) In about 24-72 hours, you will receive an email with your NPI number, print this email and SAVE IT , some insurance companies will request a copy of this email to verify that the NPI is legitimate. b) In a couple of weeks you will receive a hard copy of your number, SAVE THIS letter as well.
