Welcome to Park City Soccer Club

****Full registration must be completed by NOON on June 8, 2012 to insure player participation for Fall 2012. ****

You need to register at 2 websites in order to complete registration. Player will not be assigned to team roster until registration & payment are complete at both websites.

1. Park City Soccer Club Registration: www.parkcitysoccer.org a) Register Online tab on the left Column b) Choose the “Scholarship For All Ages” tab and begin registration c) Enter log in information (log in is your email address). If are new to the club or don’t remember your password, enter just the email address and a password will be sent to you. d) Please include and/or update contact information for both parents and player. *Cell numbers and provider and valid email are essential*. e) Submit PCSC program fees payment – in full or chose payment option. Fees will be adjusted according to your specific scholarship award. f) Follow the link to register with UYSA (instructions below). If registration is interrupted, you can find the link at the top of the registration page.

2. Utah Youth Soccer Association: **Please make note of the Team Assignment code at the top of the Tryout Results for your team, you will need this for UYSA Registration. Note that your log-in info for UYSA is not necessarily the same as for PC Soccer Club website. a) Check Player registration and follow appropriate link for Returning or new member. b) Add new and/or update family and player information c) Enter team assignment code from your tryout results page. d) DISREGARD UNIFORM ORDER INFORMATION ON UYSA. e) Continue through registration. f) On the payment screen, enter the PROMO CODE provided by scholarship committee, click verify, then add discount to order, and then Close and Refresh g) Click the NO Payment Due, Continue tab at the top and proceed. h) At the end of UYSA registration, PLEASE print the Player participation/medial waiver, sign and give to your team manager.

3. Uniform Order: The forms turned in at tryouts were for sizing information only, you must now order your uniform. a) On the scholarship registration confirmation page, select the appropriate link to order the uniform. b) Log in and update personal info if necessary and submit. c) Check sizing for all pieces, order backpack if you are a new member or need one. 3. Birth Certificates: Each player must have a COPY of their birth certificate on file with the club registrar and team manager. If you did not play with Park City Soccer Club in 2011-2012, please submit a legible copy of your player’s birth certificate to the club registrar: [email protected], FAX: 877-730-4911, or mail to PCSC PO Box 980782 Park City, UT 84098. UYSA will NOT assign players without a birth certificate.

4. Player Photo: Each player is required to have a photo on record. Most players had photos taken at tryouts. If your player did not attend tryouts or missed photo opportunities, you will be notified to submit a digital photo.
