All applications must include this completed form, a completed new individual membership form with payment, and a nomination form from the applicant’s nominator.

Applications must be sent to the SIA Nominations Committee at [email protected] or SIA 1702 – 20th Street West Saskatoon, SK. S7M 0Z9 by May 19, 2017. Nominations received after May 19 will be treated as nominations from the floor.
