When Item 14 of your IACUC Protocol Review Form indicates you may be landfilling animals or using more than one means of disposition, use this form to indicate how carcass disposition will be tracked to avoid errors. The AACUP office will notify you and the IACUC office of approval.

Protocol Title: ______Describe which animals will be incinerated or landfilled and how they will be differentiated. For example, you might be able to render control animals or animals removed from the study before being administered a hazardous agent. The animals administered the agent must be marked for incineration or landfill, eg, with a colored ear tag, permanent ear notch, or other method.
