The University has a responsibility to ensure that students recruited onto the Master of Nutrition Course are eligible for Registration with the HCPC as a Dietitian. This includes ensuring that applicants do not have a criminal conviction that precludes them from the Register. Included in this pack is a Criminal Conviction Questionnaire listing the type of convictions that might preclude you from the Register. It is important to recognise that a criminal conviction or caution is not in itself an automatic bar to enrolment, but failure to disclose could be, and may result in expulsion from the course. If you do not have any such convictions please sign and return the questionnaire to the School Office when you arrive at the University. If you do have convictions or have any queries regarding convictions please contact the University (for contact details see later).

The University will have to confirm the information supplied by you by making an application for disclosure of any information relating to criminal records to the Disclosure and Barring Service (DBS).

In order for you to be in a position to complete the disclosure form, which we will give you on arrival, please make sure that you bring the following original documentation with you:

 Passport  Driving Licence  Birth Certificate  Marriage Certificate (if appropriate)  Bank/Building Society Statement  Evidence confirming any previous address you may have held within the last five years. This may comprise Bank/Building Society Statement, Credit Card Statement, utilities bill etc.  P45 or P60 showing your National Insurance number.

You must either provide 3 documents (one from Group 1 plus any two from Groups 1 or 2)(one of which must verify current address); or 3 documents from Group 2 comprising 1 from Group 2a and 2 further documents from Group 2a or 2b (one of which must verify current address) on the list of valid documents below.

These documents will be retained by the School Office (Linda Eaves) at the School of Biosciences whilst the information that you have supplied on the form is verified. This will normally only take a couple of days. You may collect your original documentation from Linda once your form has been checked and despatched to the DBS, or you may leave it in our safekeeping until the disclosure application has been processed by the DBS. The University will pay for the disclosure to be carried out.

Please note: For applicants unable to produce a Group 1 document, the University will need to ensure an external ID validation service is used to check the applicant against their records to establish the applicant’s name and living history footprint.

On the successful completion of the Master of Nutrition Course, to become a Registered Dietitian you need to register with the Health Professions Council (HCPC). Registrants with the HCPC are specifically exempt from the Rehabilitation of Offenders Act 1974 and no Convictions or Police Cautions are ever "spent" for them. Thus, it is in your interest to declare any such incident and we will seek further advice as to whether you are likely to be precluded from registration.
