Sign In Sheets and “Participant Race/Ethnicity and Gender” Forms
Creating A Sign In Sheet--- Create a sign in sheet to fit your particular needs. Always try to capture the names of the participants and their county of residence. Beyond that, collect only what identifying information you will be using. (E-mail addresses, mailing addresses, phone numbers, etc are some that are often added to a sign in sheet). Tailor your sign in sheet to your particular event. Eliminate columns you will not be using. Do not attempt to collect demographic data on a sign in sheet. You will also be using the “participant race/ethnicity and gender” form with each of your audiences at educational programming and events. ALL events and programs should utilize a sign in sheet. Signing in is optional for the participants, but as with the “participant race and gender” form we really need to encourage their cooperation.
Exceptions: 4-H programs/events and SNAP-Ed programs/events, where all the participants have been formally enrolled in the program AND you have already collected the participant race/ethnicity and gender data during the enrollment process, DO NOT have to utilize the “participant race and gender” form at each of their gatherings. You already have that information from the enrollment process. You still, however, need to report the race/ethnicity and gender makeup as requested in the MiPRS process. If you do expect participants from the general public or participants that are not formally enrolled in your programs you should utilize the “participant race/ethnicity and gender” form for that particular event. Enrollment forms for both SNAP- Ed and 4-H request a wealth of demographic data and those have been developed by the Federal government funding and management units. You need to meet the needs of those units. At all times, however, you should utilize a sign in sheet for all of your programs and events.
Participant Race/Ethnicity and Gender Forms---This form needs to be provided to participants at ALL extension events (minus the exceptions explained above for 4-H and SNAP-Ed). The completion of the form is optional for the participants. It is not optional for us to provide it or not. It is imperative that the educator/coordinator clearly articulate the need for the information to the group. Our mission, our goals, our history all demand that we meet the needs of our entire community. The completion of this form helps us determine if we are living up to the goals/standards that we as an organization have set for ourselves. Sample Sign In Sheet
