2018 Memorial Match Play Championship Entry Form June 26th – 29th, 2018 Hawk’s Nest GC Ohio State ATI Creston-Wooster, Ohio The Memorial Entry Fee is $200 US. Make your check or money order payable to: ABG Ohio. Entry Deadline is April 15, 2018.

ABG 2018 Player Card Fee is $20 US. The ABG Player Card is a one time, once a year registration fee for all ABG tournament entry applications. If you have not paid this registration fee for 2018, make a separate check or money order payable to American Blind Golf, and send it with your entry fee and entry form. The field is limited to 7 players in each sight category on a strict first come-first accepted basis. The Defending Champions from the 2017 Match Play and the National Stroke Play in each sight category will receive an automatic entry, however all player entries and fees must be received by the entry deadline April 15, 2018

Accommodations for all players and coaches will be provided for 4 days, on June 26, 27, 28 & 29 at the Comfort Inn in Wadsworth, Ohio. If you wish to arrive early or stay late, the tournament hotel rates will be available to you. A practice round will be available on Tuesday, June 26th and the tournament will be held on Wednesday, June 27th and Thursday, June 28th. On Friday, June 29th, following the competition, the “Fore” the Warrior Blind Charity Scramble will be held. Coaches are welcome to play at a fee of $100, which will be donated, to the Wounded Warrior Project & the Adaptive Technology Scholarship Program.
