APPLICATION FOR ADMISSION TO DOCTORAL STUDIES IN THE SCHOOL OF CHURCH MUSIC SOUTHWESTERN BAPTIST THEOLOGICAL SEMINARY
A completed application for doctoral studies in the School of Church Music consists of the following items:
1. The attached application form for doctoral studies in the School of Church Music. For entrance in a fall semester the application should ideally be received by February 1 and for entrance in a spring semester by September 1.

2. Four (4) completed reference forms. The Associate Dean will request these from the references listed on your application.
3. A separate application for general admission to Southwestern Baptist Theological Seminary submitted online to the Office of Admissions (http://www.swbts.edu/apply- now).
4. Official transcripts of all graduate study, which should be submitted to the Office of Admissions as part of your general seminary application. Transcripts from foreign institutions must be evaluated by a recognized credentials evaluation service, such as World Education Services (WES), PO Box 5087, New York, NY 10274-5087.
5. Official notification of GRE General Test scores. Please schedule the GRE in time for scores to be forwarded (www.ets.org).
6. For applicants whose first language is not English, official notification of the internet- based Test of English as a Foreign Language score. Please schedule the TOEFL iBT in time for the score to be forwarded (www.ets.org). Applicants not achieving a minimum score of 100 but who have earned a degree in an accredited English-language program in a North American institution may request a waiver with supporting documentation, including an official TOEFL score. Applicants who have completed an M.M. degree at Southwestern Seminary are exempt from the TOEFL requirement.
Once all required documents are received, an interview will be scheduled with the Advanced Studies Committee of the School of Church Music, which will then act on the application. Approval from both the Admissions Office and the Advanced Studies Committee is required for acceptance into the doctoral program. Name Date of Birth
