UVa Education Benefit: FM Instructions for On-line Form 2 UVa Education Benefit: FM Instructions for On-line Form 3 2 The first screen will ask you to check all of the criteria that the course, certificate, certification program or degree program meets. As noted below, you can check all that apply. If you are taking a personal enrichment course that is not job-related, it will most likely satisfy one or both of the last two criteria on the list. Press the “Save and Continue” button at the bottom of the screen after you have checked the appropriate boxes.

The second screen will provide an opportunity to enter information for one or more classes that you wish to take. Press the “Add New Class” button to start the process.

The third screen provides a form to enter information about your classes and the funding. You should have all of this information available before you start your online application. Notice that you must provide your supervisor’s computing ID at the bottom of the form. If you are applying for more than one class, you will have the option of adding other classes after you have completed this form.

The last screen requires your agreement to the terms of the benefit. Once you have read the information, checked the box, and pressed the “Save and Continue” button at the bottom, your request will automatically be forwarded to UHR for consideration. You will receive a notification as to whether your request is approved or denied.
