On _(date of injury)_, ___(name)___ suffered a traumatic, penetrating brain injury and traumatic injury to his left arm during combat operations in FOB Kalsu, Iraq. In the ensuing years and months since that event he has substantially recovered from the physical injuries he suffered and has made good progress adapting to the severity of the head trauma injury through determination and compensation. Notwithstanding his progress, __(name)__ continues to struggle with cognitive impairments and remains dependent on a strong support network of co-workers to function effectively in the work place. His ability to independently collate and process information from multiple sources in order to make decisions poses a significant challenge and often results in frustration on his part. As an example, he has great difficulty in reading and comprehending lengthy email messages and must rely upon others to read his messages and then interpret the content and present the salient points to him in a manner he will understand. If a topic has multiple components he struggles to mentally retain them in the order of presentation and relevance. In a multi-party conversation or conference setting he becomes visibly anxious and often times disengages by leaving the room. He appears physically fit yet he has difficulty in maintaining an energy level that allows him to work a normal day. He functions best for a period of 4-6 hours provided he has the latitude to conclude his efforts when fatigue overcomes him. __(Name)___ is a fine Marine and has been a valuable asset to the __(organization)__; however, the long-term effects of his injuries are undeniable and, despite his most determined efforts, render him incapable of performing duties appropriate to his rank or military occupational specialty without considerable accommodation. In my professional opinion __(name)__ has considerable and permanent mental and physical impairments and is not fit for continued military service.
