Please note: If you are under the age of 18 a parent/guardians needs to be available during all volunteering hours. In addition, a parent signature is required for all minors that have volunteered to help with dog walking.

By submitting this form you have agreed to assist C.A.R.E. on a volunteer basis, in which interactions with pets is at the volunteers' own risk. C.A.R.E. is not liable for any damages to person(s) or property caused by any animal associated with our facility. You have also agreed to indemnify and release C.A.R.E. (Officers, Members, & Board Member) of any and all liability arising from any known, unknown, or unanticipated damages to people or property while the pet is under care/supervision of the volunteer. You are also agreeing that you are fully aware of, understand, and agree to all terms and conditions of this agreement, which is a binding contract enforceable by law.
