Martinsburg VA Medical Center Welcome to the Martinsburg VAMC. The following information is being provided to help you with the orientation process. We have designed an electronic file that will be provided to you by your school that includes all of the necessary documents. Please complete this file and return 4 weeks prior to your first day.

Registration Residents and students should report to the Education Department Conference room in Building 207B at 8:00 am on the first day of the rotation for orientation. Please send/email/fax the completed application package to the Affiliation Coordinators in the Education Dept. [email protected] or [email protected] at 4 weeks prior to orientation. Please read the “Orientation Instructions” and use the “Orientation Checklist” to ensure that you have all required documents and training complete. Once the packet is received your information will be entered into several databases. This must be done prior to scheduling your appointment for obtaining fingerprints.

Personal Identity Verification Photo identification is required to be worn at all times and be with you when you are entering and leaving the facility. A list of acceptable forms of identification is included, you must have 2 forms of identification, one being picture id such as a driver’s license. Please call the PIV office at 304-263-0811 ext. 2066 or ext. 2067 to schedule an appointment to get your fingerprints the 2nd week prior to your first day. Remember your packet must be submitted/received and processed by the Education Dept. Affiliation Coordinators prior to scheduling your appointment for fingerprints. Fingerprints can be taken at any VA as long as you call to schedule an appointment. VA locations and phone numbers can be found at http://vaww.va.gov/default.asp It is very important to tell them that the fingerprint results need to be sent to the Martinsburg VAMC and provide them with these 2 numbers: SOI VAB9 and SON 1316. If you fail to provide them with these 2 designating numbers the results will not be sent to the Martinsburg VAMC. This will result in a delay or cancellation of your rotation/training.

Required Training

All residents and students must have annual mandatory training. Follow this link for the mandatory online training and print your certificate upon completion and return with your orientation documents 4 weeks prior to your first day. www.tms.va.gov This training is mandatory for all residents and students and MUST be completed prior to your arrival. Failure to do this training prior to orientation day will result in a delay of your rotation. If you are returning and this is not your first year you can choose the online refresher course. Please follow the TMS User Instructions included in this packet.

This EMR training is a brief overview. The residents can also browse the EMR training but will be required to attend the entire 4 hr computer training class in room 3C-120 at 12:30 on that Monday. This class is only offered on Mondays at 12:30. If there is any reason you will not be starting on a Monday you will need to contact us.

Parking & Vehicle Registration Vehicles parked on the VAMC campus must be registered, with the information desk, located on the first floor of Building 500 in the main lobby on your first day. Included in your packet is MCM-07B-02 with attachment A (handicap parking request) and attachment B (campus map). A vehicle registration form is included in the packet. In order to register your vehicle you must complete the vehicle registration form and present a valid driver’s license, proof of automobile insurance and your vehicle registration card. Please have these forms complete prior to orientation. VA Policies There are several Medical Center Memorandums (MCM) included in your orientation packet. Please read all of these. If you are a medical resident please also read MCM 11-62 Monitoring of Resident Supervision and sign the signature page and return this with your orientation packet.

Library The library is located on the 2nd floor room 2B-150A. The library is open from 8:00 am to 4:30 pm Monday through Friday.
