Job posting for: ALC Plus Case Coordinator Reports to: Director of Menlo Park Academy Status: Full Time, Non-Exempt, Monday-Friday, daytime and after-school hours, occasional night/weekend hours

Broad Scope and Function of Job: The Case Coordinator will be responsible for the day to day oversight and management of assisting enrolled ALC Plus students to achieve success in reaching educational goals. The Case Coordinator will consult with the ALC Plus Educational Team, Menlo Park Staff, System Navigator, Hennepin County Probation and Court Staff and parents to assure student success in the program. Assist with problem solving student participation/attendance, behavioral issues and assist with coordination of other supportive mental/chemical health referrals services. Facilitate transition to mainstream education settings as appropriate. Transport youth as needed.

Major Job Responsibilities: Is responsible for, but not limited to the following functions:

 Provides advice, guidance and support to students, parents/guardians and other stakeholders.  Promotes positive social skills and appropriate pro-social behavior with referred students.  Consults and collaborates effectively with parents/guardians, educational staff, administrators and other educational/community resources regarding students with identified concerns and issues.  Monitors student academic performance, behavioral and attendance and assists with appropriate interventions in conjunction with the other ALC Plus team members.  Assists and supports students who are experiencing emotional, behavioral, or health related issues and/or students who have difficulty attending school on a consistent basis.  Supports, tracks and assists with problem solving issues associated with students participating/referred to the ALC Plus Program.  Provide case coordination for students referred into the ALC Plus Program.  Collects and accurately interprets, utilizes and reports student data as required.  Participates in recreational and educational after school activities for students.  Introduces and refer students to resources that may assist them with personal and educational issues.  Travels to settings where services are provided, primarily family, home, and school and community resources.  Transports program participants to educational settings, court appearances and home if necessary.  Attends on-going staff development opportunities as required by the contracting agency including mandatory trainings.  Collects and documents designated data and be able to present data in staff meetings. Prepare reports as required.  Keeps necessary case notes documenting contact with program participant, family and collateral contacts.  Assists with the identification unmet social/emotional needs of students and make recommendations/referrals for further assessments and services.  Participates in the integrated/multi disciplinary intake team to assist in the development of a complete Student Service Plan and other intake paperwork.  Participates in school staff meetings and student conferences as needed  Assists families in connection with community resources.  Develops and utilize available area resources and make appropriate referrals.  Provides an appropriate professional role model for families and encourage/support parents in the parenting of their children.  Develops professional caring relationships with program participants and families.  Assures program participant privacy is maintained by being knowledgeable and following HIPPA/FERPA data privacy and security regulations.

Job Requirements and Qualifications:

Education:  Bachelor’s Degree in Human Services or related field

Experience:  Two years experience in working with adolescents and families.  Knowledge of Education and County Probation and Human Services Systems.

Other Qualifications:  Ability to communicate effectively, both orally and in writing.  Ability to work with a diverse population of individuals by using tact diplomacy and flexibility.  Skill in dealing with parent and student concerns.  Skill in communicating with school educational staff, Hennepin County representatives.  Ability to have a flexible schedule to consistently accommodate phone and email inquiries.  Knowledge of adolescent mental and chemical health issues.  Knowledge of Hennepin County Department of Corrections and Human services systems.  Knowledge of Educational resources.  Awareness of community based mental and chemical health resources.  Ability for creative problem solving, decision making, independent judgment and negotiating/mediating.  Computer competencies, including MSOffice, databases, creating professional documents.  Ability to work across ethnic/racial/economic lines for effective communication and trust building.  Ability to handle confidential information appropriately.  Ability to self-start and effectively problem solve.  Valid Minnesota driver’s license, safe driving record and reliable transportation required

Compensation: $18.00-$20.00 an hour, depending upon qualifications. Generous benefits including Health, Dental, Life, STD & LTD, 403(b) and PTO.
