88 >100% >80, <=100 >=50, <=80 86 <50 - Mother registration (70%) on MCTS portal is less than mother registration in HMIS (97%). Child registration is also good as compared to the child registration in HMIS. Most of the 84 BCG DPT1/Pentavalent1 DPT3/Pentavalent3 Measles Full Immunization child were given BCG (95%), and 89% child received full immunization in 12 months after its regisgtration. However, full immunization in HMIS is 92% right now. It indicates poor updation on MCTS portal or rendered services. PHC Gaps in ANC Registration : 20% >100% 1. ANC registration of dist. is 97% and 6 blocks have reproted >100 % need >80, <=100 check data although AHS 2012-13 showing ANC registartion 75%. >=50, <=80 <50 SC 2- ANC Registration in 1st trim. is 52% against registration of 97% which is 70% reported less as compared to AHS 12-13 . There is opportunity gap of 50% for mother registration in 1st trimester. Also, Strategies should be planned to improve it. 3- % of ANC recievd 3 ANC checkup is 71% where as of AHS 12-13(44%). Gaps Idenified :

1. Complications are not identified in Bharaw block and Sursa has overreported Obstetric complications
