DEMOGRAPHIC OVERVIEW

The population size and boundaries for the North Community Health Partnership localities have been established in conjunction with Council and are based on local authority ‘ward’ structures. A key consideration for CHP’s is coterminosity with local authority social work and community planning boundaries to support the promotion of effective partnership working.

Motherwell locality encompasses the areas of Calder Valley, Ladywell, Forgewood, North Motherwell, Watsonville, Knowetop, Muirhouse and part of Netherton all part of North Lanarkshire Council area, and provides community based nursing, medical and paramedical care to a population of 31,899.

The most significant approach CHP localities can take to tackle health inequalities is to target and enhance primary care services in deprived areas. The Scottish index of multiple deprivation is a weighted combination of indicators used to describe various aspects of deprivation. Taken in total, these indicators provide a comprehensive picture of deprivation within small area geographies termed ’datazones’. The most deprived 15% of datazones in are the target for national regeneration funding through commu n ity planning partnerships.

Within Motherwell locality, 15 datazones fall into the worst 15% in Scotland, i.e. 27% of the locality population. This is significant to note in terms of associated health and social need. ". LIST OF GP PRACTICES

There are eight GP practices (27 GPs) within the locality boundary. Six are based in Motherwell Health Centre with the remainder having their own premises. Practice nurses are employed within the locality GP practices.

There are a range of Dental Practitioners, Pharmacists and Opticians available within the area.

Primary Care and Community services are predominantly provided from Motherwell Health Centre. There are also two Medical Centres owned and run by their respective partners. The locality has a large Community Nursing Team and a full range of Allied Health Professionals working on the basis of self managed teams.

The locality also benefits from the support and involvement of the wider Primary Care Team incorporating administrative, ancillary and technical staff.

The local population receives Acute Hospital services primarily from General Hospital (01698 361 100) and District General Hospital (01236 748748). COMMUNITY NU RS ING S ERVlCES

DISTRICT NURSES

They care manage patients with long term conditions and palliative/terminal care. They work closely with social work and the acute sector, liaising with relevant agencies. They run Nurse led clinics and at all times promote Healthy lifestyles.

The role of the Public Health Nurse is to provide targeted interventions for children and families in line with the HALL 4 recommendations on Core, Additional and Intensive needs. These interventions are around child development, immunisation and health promotion to families. The teams also have a remit for community and group work in relation to healthy lifestyle advice and intervention.

Each team is geographically aligned to work with local Communities, GP Practices, Schools and Nurseries within their specific area. Each GP practice, School and Pre-5 Nursery will have a named Public Health Nurse.

Clinical leadership of Public Health Teams is provided by a Nursing Team leader with a specialist qualification in PH Practice. The teams are accountable to the Service Development Manager for Public Health within the locality.

Public Health Teams consist of a number of staff disciplines including Public Health Nurses (health visitors and school nurses are now trained jointly to graduate as Public Health nurses), Health Promotion staff, Public Health support staff, Nursery Nurses and Health Project Workers, e.g. ‘Childsmile’ and Community Health Educators.

A Public Health Practitioner provides leadership in the development and delivery of public health programmes within the locality.

The public health teams take a broad view of health acknowledging the links between poor health and deprivation and work collaboratively with other health disciplines, external agencies and voluntary groups to develop and support health improvement activities for patients, staff and the wider comm un ity.

Targeting health improvement action and resources to our most disadvantaged communities and providing early interventions to prevent escalation of health care needs is a priority area for action.

Treatment room nurses provide care to patients attending health centres and clinics. Most referrals come for GP practices; therefore they have to establish a close working relationship with the primary health care team.

Motherwell Locality hosts the Community Dietetic Service which provides a range of services both clinically and in terms of health promotion. The team specialises in the field of chronic disease management e.g. diabetes or coronary heart disease and develop individual nutrition plans for people as part of their package of care. Staff wok closely with all members of the primary / acute care team to promote nutritional well-being and enable individuals to develop control over the many factors impacting on their health.

The Podiatry Service is provided within a variety of setting including hospitals, health centres, and residential homes and in patient's own homes. The service provides assessment, diagnosis and treatment of conditions affecting the foot and lower limb.

SPEECH AND LANGUAGE THERAPY SERVICE

The physiotherapy service aims to provide assessment, diagnosis and treatment using various skills including manual therapy, exercise and electrotherapy to treat many conditions, illnesses and injuries.

END VIOLENCE & AGGRESSION (EVA)

The EVA Project is a specialist service for women in North Lanarkshire who have experienced abuse at any time in their lives. EVA provides information and advocacy, counselling and clinical psychology services. The EVA office is based at Coathill Hospita! in Coatbridge, but counselling services are .v provided at health centres and clinics in Motherwell, Coatbridge, Airdrie and Cumbernauld. In addition, EVA staff will sometimes see women in their own homes.

DALZIEL CENTRE

The Dalziel Centre is a Day Hospice for patients diagnosed with cancer in Lanarkshire. It is based at Strathclyde Hospital in Motherwell and offers emotional psychological and social support to patients, carers and families who live with a cancer diagnosis.

The Centre helps patients live with their cancer with confidence and dignity and offers carers respite for the day while patients receive support in a safe and comfortable environment. Patients can be referred by GPs, district nurse, Macmillan nurse, hospital staff, social workers and the St Andrew’s Hospice.

ADULT MENTAL HEALTH COMMUNITY SERVICES

FOCUSED INTERVENTION TEAM

Senior Charge Nurse - Pravin Munogee (Base - Focus Day Hospital, Airbles Road, Motherwell, Tel: 01698 269336)

The Focus Intervention team provides assessment and treatment to individuals with mild to moderate individuals within the Locality Primary Care setting, who have specific and limited mental health needs between the ages of 16 - 65. Such individuals are likely to respond to focused time limited therapeutic interventions. Usual diagnosis is anxiety, depression and bereavement etc.

OUTREACH TEAM

Senior Charge Nurse - Marion Findlay (Base - Torrance House, Windmillhill Street, Mothenvell, Tel: 01698 266717)

The Outreach team is a multi disciplinary team who provides assessment and treatment to individuals within the Bellshill Locality who suffer from severe and enduring mental health problems between the ages of 16 - 65. It is envisaged that these individuals require long term care and support. Usual diagnosis is schizophrenia, manic depression and chronic depression etc.

LANARKSHIRE ALCOHOL AND DRUG SERVICE (LAADS)

Senior Charge Nurse - Jim Robertson (Base - Torrance House, Windmillhill Street, Motherwell, Tel: 01698 210054) The LAADS team is a multi disciplinary service providing assessment and treatment for individuals with a variety of addiction problems between the ages of 16 - 65. The team provide a service to individuals from Motherwell, Bellshill and Wishaw Localities.

ELDERLY COMMUNITY MENTAL HEALTH TEAM

Elderly Community Mental Health Team provides assessment and treatment to individuals within Motherwell locality who suffer from dementia and delivers Community Psychiatric Nursing service to individuals over 65 years of age.

OLDER PEOPLES DAY HOSPITAL-Airbles Road Centre

Senior Charge Nurse-Cathy Ellis Day care assessment and treatment for people with a mental health problem including a “Memory Clinic” for people with a Dementia.

CLINICAL GOVERNANCE

The team gathers evidence on changes to practice and monitors standards of care locally.

Motherwell Locality hosts and manages Clinical Governance staff for Old Age Psychiatry Pan Lanarkshire Services. Clinical Governance works collaboratively with staff across adult mental health to continuously improve the systems, structures and processes of care delivery in accordance with national and local priorities.
