Stopping the Routine use of Contact Precautions for Management of MRSA and VRE at Three Academic Medical Centers: An Interrupted Time Series Analysis 04 Jul 2020 : Contact precautions (CP) are a widely adopted strategy to prevent cross-transmission of organisms, commonly methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin- resistant Enterococcus (VRE). Some hospitals have discontinued CP for patients with MRSA or VRE; however, the impact on hospital-acquired infection rates (HAI) has not been assessed systematically. COVID -19 Infection: Strategies on When to Discontinue Isolation, a retrospective study 04 Jul 2020 COVID-19, the novel coronavirus SARS-CoV-2 infection, was first reported in Wuhan China on December 31st, 2019, and has since spread, infecting over 9.2 million people worldwide as of June 24th, 2020.1 There has been a global effort to better understand the disease. Some studies have shown patterns of transmission with viral shedding and the concern of asymptomatic transmission Drape to Prevent Disease Transmission During Endotracheal Intubation 03 Jul 2020 Endotracheal intubation poses high risk of transmission of SARS-CoV-2 and other respiratory pathogens. We designed and here describe a protective drape that we believe will greatly reduce this risk. Unlike the intubation box that has been described prior, it is portable, disposable, and does not restrict operator dexterity What is the best timing for healthcare workers infected COVID-19 to return to work? 03 Jul 2020 The current COVID-19 pandemic has exposed the healthcare system to its greatest risk, evident with a rapid depletion of medical resources and draining of intensive care units’ capacity.1 Nosocomial transmissions, especially healthcare workers (HCWs) getting infected with COVID-19 in healthcare facilities, have been reported by many countries, for example, 20% in Lombardy (Italy) and 26% in Spain, Navigating Reflex Urine Culture Practices in Community Hospitals: Need for a Validated Approach 03 Jul 2020 We performed a descriptive study of reflex urine culture (RUC) practices across 51 community hospitals in southeastern United States. We found that 26 unique reflexing criteria were used in 28 hospitals. Only 14% hospitals of hospitals that offered RUC restricted it to specific populations (e.g. emergency room) Hospital affiliated long term care facility COVID-19 containment strategy by using prevalence testing and infection control best practices 02 Jul 2020 In the United States, the elderly population is the most vulnerable and likely to succumb to complications and die of COVID-19. We have seen multiple reports of deaths due to a COVID-19 outbreak in group living facilities How Common Are Indwelling Devices in Hospitalized Adults? A Contemporary Point Prevalence Study in A Tertiary Care Hospital 01 Jul 2020 Healthcare associated infections (HAI) are associated with indwelling devices. Yet, data regarding prevalence of indwelling devices in non-critically ill hospitalized patients remains scant. Protection Challenges of Pregnant Women against Vertical Transmission during COVID-19 Epidemic: A Narrative Review 01 Jul 2020 Vulnerable populations require intensive care during the outbreak of communicable diseases. Extensive measures are undertaken to ensure that the vulnerable population gets the appropriate protection and treatment. Pregnant women are a crucial part of the vulnerable population Resumption of Respiratory Outpatient Services in the COVID-19 era: experience from Southern Italy 01 Jul 2020 COVID-19 pandemic turned the entire health-care system organization upside-down, suspending elective activities and outpatient services. In Italy, we are entering a second phase of the pandemic and several strategies has been developed to “re-open” the country, some businesses, and also health-care outpatient activities Impact of SARS-CoV-2 on Hospital Acquired Infection Rates in the United States: Predictions and Early Results 01 Jul 2020 The first case of SARS-CoV-2, also known as COVID-19, in the United States was reported on 1/22/2020,1 and the disease continues to spread throughout the United States relatively quickly. Even before cases were confirmed in the U.S., many Infection Preventionists (IP) were involved in preparation activities for their facilities Variability in infection surveillance methods and impact on surgical site infection rates 01 Jul 2020 Surgical site infections (SSI) are among the most common and costly hospital-acquired infections, associated with increased morbidity, length of hospital stay, re-admissions and healthcare costs.1-4 SSI prevention is a high priority for multiple stakeholders, including patients, clinicians, hospitals, payers, and regulatory agencies. An Electronic Questionnaire to Survey Colorectal Cancer Screening Status and Identify High-Risk Cohorts in Large Health Care Organizations 01 Jul 2020 American Journal of Medical Quality, Ahead of Print. Though improved screening practices have reduced the incidence and mortality of colorectal cancer (CRC), screening rates continue to be suboptimal. This is especially true of high-risk individuals, who are difficult for clinicians to identify during a typical health care encounter Key Features Underlying Low-Value Care Recommendations 30 Jun 2020 American Journal of Medical Quality, Ahead of Print. One potential reason that low-value care remains persistent is variation in recommendations created to deter it. A better understanding of key features, and how they differ across a range of recommendations, can offer insight about improvement opportunities [back to top]

Infections in Patients Colonized with Extended-Spectrum Beta- Lactamase-Producing Enterobacterales – a Retrospective Cohort Study 01 Jul 2020 AbstractWe investigated relative proportions of extended-spectrum beta-lactamase- producing Enterobacterales (ESBL-PE) versus non-ESBL-PE (nESBL-PE) infections in ESBL-PE colonized patients. ESBL-PE are not causative for the majority of infections in hospitalized patients colonized with ESBL-PE Impact of SARS-CoV-2 Viral Load on Risk of Intubation and Mortality Among Hospitalized Patients with Coronavirus Disease 2019 30 Jun 2020 AbstractBackgroundPatients hospitalized with coronavirus disease 2019 (COVID-19) frequently require mechanical ventilation and have high mortality rates, but the impact of viral burden on these outcomes is unknown.MethodsWe conducted a retrospective cohort study of patients hospitalized with COVID-19 from March 30 to April 30, 2020 at two hospitals in New York City Outcomes of reflex cryptococcal antigen (CrAg) screening in HIV- positive patients with CD4 counts of 100-200 cells/µL in Botswana 30 Jun 2020 AbstractIncreasing the CD4-count threshold for cryptococcal antigen (CrAg) screening from ≤100 to ≤200 cells/µL resulted in a 3-fold increase in numbers screened. CrAg-prevalence was 3.5% at CD4 101-200 and 6.2% ≤100 cells/µL. Six-month mortality was 21.4% (9/42) in CrAg-positive CD4 ≤100 cells/µL and 3.2% (1/31) in CrAg-positive CD4 101-200 cells/µL. Use of Baricitinib in Patients with Moderate and Severe COVID-19 29 Jun 2020 AbstractCytokine storm and hyperinflammation are associated with increased mortality in COVID-19. In this small uncontrolled cohort of patients with moderate-severe COVID-19, treatment with baricitinib plus hydroxychloroquine was associated with recovery in 11 of 15 patients. Baricitinib for the treatment of COVID-19 should be further investigated in randomized controlled clinical trials. Comparing Nasopharyngeal and Mid-Turbinate Nasal Swab Testing for the Identification of SARS-CoV-2 29 Jun 2020 AbstractTesting of paired mid-turbinate (MT) nasal and nasopharyngeal (NP) swabs, collected by trained personnel from 40 patients with COVID-19 showed more NP (76/95, 80%) than MT swabs tested positive (61/95, 64%; p=0.02). Among samples collected a week after study enrollment, fewer MT than NP samples were positive (45% vs 76%; p=0.001). COVID -19 pharmacologic treatments for children: research priorities and approach to pediatric studies 29 Jun 2020 AbstractClinical trials of pharmacologic treatments of coronavirus disease 2019 (COVID-19) are being rapidly designed and implemented in adults. Children are often not considered during development of novel treatments for infectious diseases until very late Distribution of Transmission Potential during Non-Severe COVID-19 Illness 29 Jun 2020 AbstractBackgroundPatients recovering from coronavirus disease 2019 (COVID-19) often continue to test positive for the causative by PCR even after clinical recovery, thereby complicating return-to-work plans. The purpose of this study was to evaluate transmission potential of COVID-19 by examining viral load with respect to time SARS -CoV-2 infection in healthcare personnel with high-risk occupational exposure: evaluation of seven-day exclusion from work policy 29 Jun 2020 ABSTRACTBackgroundStarting in late February 2020, Greece is experiencing a coronavirus disease 2019 (COVID-19) epidemic. Healthcare personnel (HCP) were disproportionately affected, accounting for approximately 10% of notified cases. Exclusion from work for 7 days was recommended for HCP with high-risk occupational exposure Social distancing alters the clinical course of COVID-19 in young adults: A comparative cohort study 29 Jun 2020 AbstractBackgroundSocial distancing and stringent hygiene seem effective in reducing the number of transmitted virus particles, and therefore the infectivity, of coronavirus disease 2019 (COVID-19) and could alter the mode of transmission of the disease. However, it is not known if such practices can change the clinical course in infected individuals [back to top]
