Design: We conducted a randomized, parallel, open-label, clinical trial comparing COC (30 μg of ethinylestradiol and 150 μg of desogestrel) plus spironolactone (100 mg/day) with metformin (850 mg b.i.d.) for one year in women with PCOS (EudraCT2008–004531–38). Methods: The composite primary outcome included efficacy (amelioration of hirsutism, androgen excess and menstrual dysfunction) and cardiometabolic safety (changes in the frequencies of disorders of glucose tolerance, dyslipidemia and hypertension). A complete anthropometric, biochemical, hormonal and metabolic evaluation was conducted every three months and data were submitted to intention-to-treat analyses. The naturally occurring ligand 5a-DHT has higher binding af®nity at the androgen receptor than the non-steroidal antiandrogens. Bicalutamide has an af®nity two to four times higher than 2-hydroxy¯utamide, the active metabolite of ¯utamide, and around two times higher than nilutamide for wild-type rat and human prostate androgen receptors. Animal studies have indicated that bicalutamide also exhi- bits greater potency in reducing seminal vesicle and ventral prostate weights and inhibiting prostate tumour growth than ¯utamide. Although preclinical data can give an indication of the likely clinical activity, clinical studies are required to determine effective, well-tolerated dosing regimens. As components of combined androgen blockade (CAB), controlled studies have shown survival bene®ts of ¯utamide plus a luteinising hormone-releasing hormone analogue (LHRH-A) over LHRH-A alone, and for nilutamide plus orchiectomy over orchiectomy alone. Other studies have failed to show such survival bene®ts, including those comparing ¯utamide plus orchiectomy with orchiectomy alone, and nilutamide plus LHRH-A with LHRH-A alone. -

Exposure to the Antiandrogen Flutamide

Exposure of rats in utero to the anti-androgen flutamide resulted in feminization of the external genitalia that was noticeable at birth. This exposure also resulted in a high degree of cryptorchidism during adulthood. In most affected animals, testes were lying in 'ovary position' close to the caudal pole of the ipsilateral kidney. Cryptorchidism occurred despite normal prenatal development of the gubernacular cones and the transformation of these structures, postnatally, into muscular cremaster sacs. Inter- and intralitter variation in the response to prenatal exposure to flutamide was observed as well as intra-individual variation. Cryptorchidism frequently occurred unilaterally with right side cryptorchidism predominating. Cryptorchidism occurred in association with marked suppression of the growth of the ipsilateral epididymis and deferent duct. The possibility is considered that the poor development or absence of these structures contributes to cryptorchidism. Intra\x=req-\ individual variation supports the concept of the local nature of the influence of testis hormones in stabilization and further differentiation of the ipsilateral Wolffian duct derivatives. Cryptorchidism was enhanced when rats were treated postnatally with testosterone or oestradiol. The effect of testosterone was unexpected in view of the generally held hypothesis that androgens enhance testis descent. The effect of oestradiol was as expected: other animal models have been described in which induction of cryptorchidism by oestradiol occurs. Additional treatment with oestradiol caused further suppression of growth of the epididymis and deferent duct. -

Co., Ltd., Kawasaki213, Japan Abstract. TZP-4238 is a new potent, orally active steroidal antiandrogen. Antiandrogenic activity and endocrinological profile of TZP-4238 were investigated in rats, except that progestational activity was determined in rabbits. TZP-4238 suppressed the testosterone propionate-induced increases in the weights of the ventral prostate, seminal vesicle and levator ani in castrated immature male rats. TZP- 4238 also decreased the weights of the ventral prostate, seminal vesicle and levator ani in intact adult male rats, but did not affect the weight of the testis or the serum concentrations of luteinizing hormone and testosterone. TZP-4238 did not have such an inhibitory effect on the weight of the adrenal gland as seen in other steroidal antiandrogens. It exhibited potent progestational activity. Although TZP-4238 did not exert androgenic or estrogenic activity, it had weak antiestrogenic activity. These results suggest that TZP-4238 exerts an antiandrogenic effect on the prostate without any compensatory change in the serum concentration of luteinizing hormone or testosterone in rats, and it is a useful drug for the treat- ment of androgen-dependent diseases such as prostatic hypertrophy and prostatic cancer. Date of Preparation: 10761 – 25th NE December 20, 2010 Suite 110, Building “B” Calgary, Alberta T2C 3C2 Canada Control No. 143756 PRODUCT MONOGRAPH PrAVA-BICALUTAMIDE (bicalutamide) 50mg Tablets Non-Steroidal Antiandrogen CLINICAL PHARMACOLOGY AVA-BICALUTAMIDE (bicalutamide) is a non-steroidal antiandrogen, devoid of other endocrine activity. Bicalutamide competitively inhibits the action of androgens by binding to cytosol androgen receptors in target tissue. This inhibition results in regression of prostatic tumours. AVA- BICALUTAMIDE is a racemate and the (R)-enantiomer is primarily responsible for the antiandrogenic activity of AVA-BICALUTAMIDE. Pharmacokinetics and Metabolism The absorption, distribution, metabolism and excretion of bicalutamide has been investigated after administration of a single 50 mg oral dose to volunteers. The results indicated that the dose was extensively absorbed and was excreted almost equally in urine (36%) and faeces (43%) over a 9 day collection period. There is no evidence of any clinically significant effect of food on bioavailability. Steady state plasma concentrations of the (R)-enantiomer of approximately 9 Fg/ml are observed during daily administration of 50 mg doses of bicalutamide. At steady state, the active (R)-enantiomer accounts for 99% of the circulating plasma bicalutamide concentration. 2 Bicalutamide is highly protein bound (racem ate 96%, R-enantiomer 99.6%). On daily administration, the (R)-enantiomer accumulates about 10-fold in plasma, consistent with an elimination half-life of approximately one week. The (S)-enantiomer is very rapidly cleared relative to the (R)-enantiomer. The pharmacokinetics of the (R)-enantiomer are unaffected by age, renal impairment or mild to moderate hepatic impairment. -

Sawyers, MD Date Dissertation Mentor Copyright by Minna Delarae Balbas 2013 Abstract Androgen receptor (AR) signaling plays a crucial role in the growth and progression of prostate cancer. Current therapies for prostate cancer rely on this dependence and aim to disrupt AR signaling through androgen deprivation and the use of AR antagonists (antiandrogens). While this is usually an effective treatment initially, most prostate tumors eventually regain the ability to grow. Reactivation of AR signaling has been implicated in resistance, often through overexpression or mutation of the androgen receptor. Several AR mutations have been described that broaden ligand specificity or convert AR antagonists into agonists of the mutant receptor. These mutations alter the conformation of antiandrogen-bound AR, such that the antiandrogen now activates the receptor. The second-generation antiandrogen enzalutamide (formerly MDV3100) recently received FDA approval for patients with castration resistant prostate cancer. Despite its success, the duration of patient response is often limited. To prospectively identify AR mutations that might confer resistance to enzalutamide, we developed and performed a reporter-based mutagenesis screen using an AR-regulated EGFP reporter and randomly mutagenized AR cDNA library. After several rounds of enzalutamide exposure and FACS- sorting, we enriched a population of cells that maintain EGFP expression in the presence of drug. -
